集束化管理在降低外周血管介入手术后并发症中的应用价值

Application Value of Centralized Management in Reducing Complications after Peripheral Vascular Intervention Surgery

目的:探讨集束化管理在降低外周血管介入手术后并发症中的应用价值。方法:本院于2017年5月开始实施集束化护理管理模式,将2016年5月-2017年4月实施常规护理方案干预的外周血管介入手术患者50例纳入对照组,将2017年5月-2018年4月实施集束化管理模式护理干预的外周血管介入手术患者53例纳入观察组。比较两组止血相关事件、血管相关并发症、术后并发症发生情况及干预前后的心理状态(SAS、SDS)变化情况。结果:观察组止血相关事件发生率为16.98%,低于对照组的46.00%,差异有统计学意义(P<0.05);观察组血管相关并发症发生率为5.66%,低于对照组的22.00%(P<0.05);两组术后腹胀、食欲下降发生率比较,差异均无统计学意义(P>0.05),观察组术后腰背酸痛、术肢麻木发生率分别为5.66%、5.66%,均低于对照组的20.00%、22.00%,差异均有统计学意义(P<0.05);干预后,观察组SAS、SDS评分均显著低于对照组,差异均有统计学意义(P<0.05)。结论:集束化管理可有效改善外周血管介入手术患者术后相关并发症发生情况,改善腰背酸痛、术肢麻木状态,缓解术后焦虑、抑郁情绪,效果显著。

Objective:To explore the application value of cluster management in reducing complications after peripheral vascular intervention surgery.Method:In May 2017,our hospital began to implement the centralized nursing management mode,of which 50 patients with peripheral vascular intervention surgery whose implemented the routine nursing program intervention from May 2016 to April 2017 into the control group,and 53 patients with peripheral vascular intervention surgery whose implemented the centralized management mode nursing intervention from May 2017 to April 2018 into the observation group.The hemostasis related events,vascular related complications,postoperative complications and the changes of psychological state(SAS,SDS)before and after intervention were compared between the two groups.Result:The incidence of hemostasis related events in the observation group was 16.98%,lower than 46.00% in the control group(P<0.05).The incidence of vascular related complications in the observation group was 5.66%,lower than 22.00% in the control group(P<0.05).There were no significant differences in the incidence of abdominal distention and appetite decline between the two groups(P>0.05),the incidence of postoperative lumbago and numbness in the observation group were 5.66% and 5.66%,respectively,which were lower than 20.00% and 22.00% in the control group,the differences were statistically significant(P<0.05).After intervention,the scores of SAS and SDS in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Centralized management can effectively improve the postoperative complications of patients with peripheral vascular intervention,improve the state of back pain and limb numbness,and relieve the postoperative anxiety and depression.
