鹰潭地区胃癌新发病例人群分布及HER-2表达情况与预后的相关性研究

Study on Population Distribution of New Gastric Cancer Cases in Yingtan Area and Correlation between HER-2 Expression and and Prognosis

目的:探讨鹰潭地区胃癌新发病例人群分布及HER-2表达情况与预后的相关性。方法:选取2015年1月-2017年12月本院收治行胃癌根治术的121胃癌患者作为研究组,选取30例胃良性病变患者作为对照组。应用免疫组织化学方法对研究组中的109例胃癌标本进行HER-2基因的表达,并将检测结果与患者的随访资料及临床病理参数进行综合分析。分析胃癌组织HER-2表达状况、HER-2表达与生物学和胃癌临床行为关系、HER-2表达与预后相关性。结果:研究组中12例未进行HER-2表达检测,对照组患者HER-2表达全部为阴性,胃癌组织已行HER-2检测的109例患者,其中HER-2阳性表达例数为15例,阳性率为13.76%,两组阳性率比较差异有统计学意义(P<0.05);胃癌组织中HER-2表达与有无远处转移、Lauren分型、肿瘤部位、浸润深度、分化程度均存在明显差异(P<0.05),与临床TNM分期、性别、有无淋巴结转移和年龄均无明显差异(P>0.05);研究组HER-2过表达患者9例,阴性表达者中位生存时间明显长于过表达者(P<0.05)。结论:有无远处转移、Lauren分型、肿瘤部位、浸润深度、分化程度与HER-2是否表达均存在相关性,可对患者预后和生物学行为进行有效判断。对HER-2是否表达和预后探究具有较高临床价值,为临床治疗提供了有效理论依据,可根据患者病情制定合理治疗方案,进一步促进病情恢复。

Objective:To investigate the population distribution of new gastric cancer cases in Yingtan area,ant the correlation between HER-2 expression and prognosis.Method:A total of 121 gastric cancer patients admitted to our hospital from January 2015 to December 2017 were selected as the research group,and 30 patients with benign gastric lesions were selected as the control group.Immunohistochemistry was used to detect the expression of HER-2 gene in 109 gastric cancer specimens.The expression status of HER-2 in gastric cancer tissues,the relationship between HER-2 expression and biological and clinical behavior of gastric cancer,and the correlation between HER-2 expression and prognosis were analyzed.Result:In 121 patients,12 patients did not do HER-2,the control group was negative for HER-2,109 patients with gastric cancer,15 patients with HER-2 positive,the positive rate was 13.76%,there was a significant difference between two groups(P<0.05).High expression of HER-2 protein in gastric cancer woven with and without distant metastasis,Lauren classification,tumor location,depth of invasion,degree of differentiation were significantly different(P<0.05),and clinical TNM staging,gender,presence or absence,lymph node metastasis and age,there were no significant differences(P>0.05).Among 121 patients with gastric cancer,9 patients with HER-2 overexpression,the median survival time of the negative patients was significantly higher than that of the overexpression patients(P<0.05).Conclusion:The presence or absence of distant metastasis,Lauren typing,tumor site,invasion depth and differentiation degree are correlated with HER-2 gene,which can effectively determine the prognosis and biological behavior of patients.The exploration of HER-2 gene expression and prognosis has a high clinical value,which provides an effective theoretical basis for clinical treatment of patients,and a reasonable treatment plan can be developed according to the patient’s condition,further promoting the recovery of the disease.
