中药熏洗配合康复运动护理对2型糖尿病周围神经病变的临床价值分析

Clinical effect of Chinese herbal fumigation combined with rehabilitation exercise nursing on patients with type 2 diabetic peripheral neuropathy

目的评估中药熏洗结合康复运动训练对糖尿病周围神经病变(DPN)患者的腓肠感觉和腓总运动神经传导速度的影响。方法选取2017年10月~2018年12月在本院接受治疗的87例伴有DPN的2型糖尿病患者作为研究对象。通过计算机生成随机数表将患者随机分为实验组(中药熏洗+康复运动训练+标准治疗,n=40)和对照组(标准治疗,n=47)。分别在基线和干预后第8 w进行电生理评估和MDNS测量。结果对照组和实验组的MDNS基线得分分别为(13.55±1.75)分和(12.57±1.74)分;治疗8 w后,对照组和实验组的MDNS平均分分别为(14.57±1.50)分和(7.03±1.86)分,两组治疗期间MDNS总分变化有显著差异(F=54.042,P<0.001)。在治疗期间,两组在腓总运动神经远端传导速度和腓肠感觉神经传导速度的变化存在显著差异(P<0.05)。结论对糖尿病DPN患者在常规治疗基础上予以中药熏洗联合康复运动护理,可进一步提高患者神经功能恢复,促进疗效,值得临床选择。

Objective To evaluate the therapeutic effect of combining traditional Chinese medicine fumigation with rehabilitation exercise on nerve conduction velocity of sural sensory and peroneal motor nerve in diabetic peripheral neuropathy(DPN).Methods 87 patients with type 2 diabetes with DPN admitted to the hospital from October 2017 to December2018 were included in this study.The patients was randomly assigned to experimental(Chinese medicine fumigation+rehabilitation exercise+standard care,n=40)and control groups(standard care,n=47)by computer generated random number tables.The electrophysiology and MDNS scores of patients at baseline and 8 weeks after intervention were measured.Results MDNS baseline scores were(13.55±1.75)points in the control group and(12.57±1.74)points in the experimental group.After 8 weeks of treatment,the average MDNS scores of the control group and the experimental group were(14.57±1.50)and(7.03±1.86)respectively.Total scores showed a significant difference for two groups with an F=54.042 and P<0.001.During the treatment period,there was a significant difference in the two groups for conduction velocity of the distal segment of peroneal nerve and sacral sensory nerve(P<0.05).Conclusion The traditional Chinese medicine fumigation and washing combined with rehabilitation exercise nursing on the basis of routine treatment of DPN patients with diabetes can further improve the neurological function recovery of patients and promote the curative effect,which is worthy of clinical selection.
