ERAS措施在胃癌患者腹腔镜围手术期的应用

The Curative Effect Evaluation of Enhanced Recovery after Surgery for Patients with Laparoscopic Radical Gastrectomy

目的:探讨加速康复外科(ERAS)措施在腹腔镜胃癌患者围手术期的应用效果。方法:选取110例择期胃癌手术患者,随机分为2组,对照组59例,观察(ERAS)组51例。对照组采取传统的围手术期处理方法,给予常规护理;观察组应用ERAS理念行围手术期处理。比较2组患者手术指标及术后首次排气时间、术后下床时间、术后住院时间、拔尿管时间、早期进食时间、进半流食饮食时间、并发症、住院费用。结果:观察组术后下床活动时间[(8.51±2.13)h]、术后首次排气时间[(65.71±3.54)h]、术后拔尿管时间[(23.84±5.25)h]、早期进食时间[(5.11±1.37)d]、进半流食饮食时间[(2.35±0.37)d]、术后住院时间[(6.17±1.24)d]均短于对照组;观察组住院费用[(41133.23±5194.81)元]少于对照组[(43360.25±2135.37)元];对照组有2例并发症,观察组无并发症。结论:ERAS措施应用于腹腔镜胃癌患者围手术期,可减少术后并发症、缩短住院时间、降低住院费用,加快患者康复进程。

Objective:To investigate the clinical application effects of enhanced recovery after surgery(ERAS)during perioperative period on patients undergoing laparoscopic radical gastrectomy.Methods:A total of 110 patients undergoing elective gastric cancer surgery were randomly divided into 2 groups,59 in the control group,and 51 in the observation(ERAS)group.The control group was treated with traditional perioperative treatment and given routine nursing care;the observation group applied ERAS concept for perioperative treatment.The surgical indexes and postoperative first exhaust time,postoperative bedtime,postoperative hospitalization time,urinary catheterization time,complications,and hospitalization costs were compared between the two groups.Results:The catheter time[(8.51±2.13)h],the time to first flatus[65.71±3.54)h],time to a semi-liquid diet[(2.35±0.37)d],postoperative length of stay[(6.17±1.24)d]in the observation group were all shorter than those in the control group.The hospitalization expenditure in the observation group[(41133.23±5194.81)Yuan]was lower compared with those in the control group[(43360.25±2135.37)Yuan].There were two patients had postoperative complications in the control group and no postoperative complications in the observation group.Conclusion:ERAS measures applied to patients undergoing laparoscopic gastric cancer during the perioperative period can reduce postoperative complications,shorten the length of hospital stay,reduce the cost of hospitalization,and accelerate the rehabilitation process of patients.
