四维彩色多普勒血流成像技术在诊断胎儿主动脉弓及其分支异常中的应用价值分析 被引量:4

摘要 目的探讨四维彩色多普勒血流成像技术在诊断胎儿主动脉弓及其分支异常中的应用价值。方法选取2017年3月至2019年6月在我院进行产前胎儿畸形筛查的孕妇107例为研究对象,均行四维彩色多普勒血流成像技术,并以随访结果为标准,统计其诊断结果及价值。结果经随访发现,107例孕妇中发生胎儿主动脉弓及其分支异常67例,发生率为63%;四维彩色多普勒血流成像技术诊断胎儿主动脉弓及其分支异常的准确度为89%,特异度为80%,敏感度为94%,阳性预测值为97%,阴性预测值为76%;四维彩色多普勒血流成像诊断主动脉狭窄、主动脉弓离断及血管环畸形的检出率与随访结果对比,差异无统计学意义(P>0.05)。结论四维彩色多普勒血流成像诊断胎儿主动脉弓及其分支异常的诊断价值较高,有助于产前筛查胎儿畸形,为干预措施的制定提供依据。 Objective To investigate the diagnostic value of 4D color Doppler flow imaging in anomaly of fetal aortic arch and its branch arteries.Methods Included in this study were 107 pregnant women who received prenatal screening for fetal anomaly in our hospital between March 2017 and June 2019.All subjects underwent 4D color Doppler flow imaging.The results and diagnostic value of this technique were analyzed based on follow-up data.Results During the follow-up,67 of 107 pregnant women were identified to have anomaly of fetal aortic arch and its branch arteries,with the incidence rate being 63%.The 4D color Doppler flow imaging yielded an accuracy of 89%,specificity of 80%,sensitivity of 94%,positive predictive value of 97%,and negative predictive value of 76%in diagnosing anomaly of fetal aortic arch and its branch arteries.The rates of detection foraortic stenosis,aortic arch dissection and aortic ring malformation by 4D color Doppler flow imaging did not differ significantly from outcomes of follow-up(P>0.05).Conclusion Favorable diagnostic value of 4D color Doppler flow imaging in diagnosing anomaly of fetal aortic arch and its branch arteries is helpful for prenatal screening of fetal malformations and provides evidence for development of intervention measures.
