血浆hsa-miR-19a-3p表达与急性心肌梗死的相关性研究 被引量:2

摘要 目的探讨血浆hsa-miR-19a-3p与急性心肌梗死(AMI)的相关性。方法纳入45例AMI病人作为试验组(AMI组),45例稳定型冠心病(SCAD)病人作为对照组(SCAD组)。采用实时荧光定量(qRT-PCR)检测血浆hsa-miR-19a-3p的表达,采用酶联免疫吸附法(ELISA)检测心肌标志物的表达,分析各项检测指标与AMI的相关性。结果两组病人在年龄、性别和吸烟史等基线资料比较差异无统计学意义(P>0.05),AMI组血脂水平和传统心肌标志物较SCAD组升高(P<0.05);AMI组血浆hsa-miR-19a-3p水平为(0.20±0.02),较SCAD组的(0.05±0.01)明显升高(P<0.05);血浆hsa-miR-19a-3p的表达水平与血浆肌酸激酶同工酶(CK-MB)的Pearson相关系数为0.4881(P<0.0001),与肌红蛋白(MYO)的Pearson相关系数为0.4570,与肌钙蛋白I(cTnI)的Pearson相关系数为0.4522(P<0.0001);血浆hsa-miR-19a-3p的AUC为0.8953(P<0.0001),CK-MB的AUC为0.8548(P<0.0001),MYO的ROC曲线AUC为0.9323(P<0.0001),cTnI的ROC曲线的AUC为0.9486(P<0.0001);经皮冠状动脉介入治疗(PCI)术前血浆hsa-miR-19a-3p的表达(0.2457±0.0281)和术后1 d(0.1950±0.0261)比较差异无统计学意义(P>0.05),而术后3 d血浆hsa-miR-19a-3p的表达(0.0711±0.0088)较术前明显下降(P<0.05);三支病变病人血浆中hsa-miR-19a-3p表达为(0.1304±0.0073),明显比双支病变(0.2133±0.0384)和单支病变(0.2698±0.0514)病人升高,具有统计学意义(P<0.0001);而且双支病变病人hsa-miR-19a-3p表达水平与单支病变病人比较也明显升高(P<0.0001)。结论hsa-miR-19a-3p可能与AMI发病有关,可以作为AMI发生的诊断性生物标志物,而且hsa-miR-19a-3p的表达水平可能与心肌梗死程度有关。 Objective To study the relationship of hsa-miR-19a-3p and acute myocardial infarction(AMI).Methods Forty-five patients with AMI were recruited as the test group(AMI group),and 45 patients with stable coronary artery disease(SCAD)as the control group(SCAD group).The plasma hsa-miR-19a-3p level was detected by quantitative real time polymerase chain reaction(qRT-PCR).Enzyme-linked immunosorbent assay(ELISA)was used to detect the expression of myocardial markers.The correlation between each detection index and AMI was analyzed.Results There was no significant difference in the basic data such as the age,gender and smoking history between the two groups of patients(P>0.05).The levels of blood lipid and traditional myocardial markers were higher in the AMI group than that in the SCAD group(P<0.05).The level of plasma hsa-miR-19a-3p in the AMI group was significantly higher than that in the SCAD group[(0.20±0.02)vs(0.05±0.01),P<0.05].The Pearson correlation coefficient between plasma hsa-miR-19a-3p expression level and creatine kinase MB(CK-MB),myoglobin(MYO),troponin I(cTnI)was 0.4881,0.4570,0.4522,respectively(P<0.0001).The area under the curve(AUC)of receiver-operating characteristic(ROC)of plasma hsa-miR-19a-3p,CK-MB,MYO,and cTnI was 0.8953,0.8548,0.9323,and 0.9486,respectively(P<0.0001).Plasma hsa-miR-19a-3p expression before PCI(0.2457±0.0281)and 1 d after surgery(0.1950±0.0261)was not statistically significant(P>0.05),and plasma hsa-miR-19a-3p expression(0.0711±0.0088)decreased significantly at 3 d after PCI(P<0.05).The expression of plasma hsa-miR-19a-3p(0.1304±0.0073)in patients with three vessel lesions was significantly higher than that in patients with two vessel lesions(0.2133±0.0384)or single vessel lesions(0.2698±0.0514,P<0.0001).The expression of plasma hsa-miR-19a-3p was also significantly higher in patients with two vessel lesions compared with patients with single vessel lesions(P<0.0001).Conclusion The hsa-miR-19a-3p may be related to the pathogenesis of AMI,and can be used as a diagnostic biomarker for AMI,and the expression level of hsa-miR-19a-3p may be related to the degree of myocardial infarction.
