联合检测HE4、β-HCG、DTI对卵巢肿瘤的诊断价值

The value of combined detection of HE4、β-HCG and DTI in the diagnosis of ovarian tumors

目的探讨联合检测人附睾蛋白4(HE4),人绒毛膜促性腺激素(p-HCG)、弥散张量成像(DTI)对卵巢肿瘤的诊断价值。方法选择2017年5月一2019年5月在西安市第四医院妇科就诊的盆腔包块且拟行手术的患者100例作为研究对象,根据术后病理报告分为良性组48例和卵巢癌组52例,对比两组患者DTI数据及HE4、β-HCG表达水平,单独检测DTI、HE<、p-HCG以及联合检测阳性率,同时分析以上检测方法对卵巢癌的诊断价值。结果良性组HE4、β-HCG表达水平明显低于卵巢癌组,良性组表观扩散系数(ADC值)、扩散系数(D)值及各向异性分数(FA值)均明显高于卵巢癌组,差异均具有统计学意义(PV0.05)。联合检测阳性率高于DTI、HE4、β-HCG单独检测,联合检测和单独检测DTI、HE<、(3-HCG AUC(曲线下面积)分别为0.950.0.934,0.918.0.838,联合检测灵敏度和特异度最高。结论联合检测HE4、β-HCG、DTI对诊断卵巢肿瘤具有较高的临床应用价值。

Objective To investigate the value of combined detection of HE4,{3-HCG and DTI in the diagnosis of Ovarian tumors.Methods We selected 100 patients with pelvic mass from May 2017 to May 2019 who were treated in the department of Gynaecology and were scheduled for operation.According to the postoperative pathological report,48 patients were divided into benign group and 52 patients were divided into ovarian cancer group The DTI data and the expression of HE4,R-HCG were compared between the two groups.The positive rates of DTI,HE4,β-HCG and combined detection were detected separately.Results The expression of HE4,(3-HCG in benign group was significantly lower than that in ovarian cancer group,and the ADC value,D value and FA value in benign group were significantly higher than that in Ovarian Cancer Group(P<0.05)・The positive rate of DTI,HE4,0-HCG were higher than those of single test.DTI,HE4,β-HCG AUC(area under curve)were 0.950,0.934,0.918 and 0.838,respectively. The joint test had the highest sensitivity and specificity. Conclusion The combined detection of HE4 ,β-HCG and DTI has high clinical value in the diagnosis of ovarian tumors.
