无创机械通气治疗老年急性左心衰并呼吸衰竭的效果及对患者生活质量的影响

Effect of Noninvasive Mechanical Ventilation in Elderly Patients with Acute Left Heart Failure Associated with Respiratory Failure and Its Effect on Quality of Life

目的:总结分析无创机械通气治疗老年急性左心衰并呼吸衰竭的临床效果。方法:选择2015年8月~2018年8月期间某院收治的90例老年急性左心衰并呼吸衰竭患者为研究对象,随机分为观察组和对照组各45例。对照组应用常规治疗方案,观察组在常规治疗基础上应用无创机械通气疗法,观察比较两组患者临床疗效。结果:观察组呼吸困难缓解率及临床治疗总有效率明显高于对照组,而住院时间低于对照组;观察组PO2明显高于对照组,PCO2、RR明显低于对照组;随访1个月,观察组SF-36生活治疗评估得分明显高于对照组,组间差异显著(P<0.05)。结论:无创机械通气在老年ALHF伴呼吸衰竭治疗中有助于缓解呼吸困难症状,改善血气指标,提高临床治疗效果及生活质量水平。

Objective:To summarize and analyze the clinical effect of noninvasive mechanical ventilation in elderly patients with acute left heart failure associated with respiratory failure.Methods:90cases of elderly patients with acute left heart failure and respiratory failure admitted to a hospital from August 2015to August 2018were selected as the study subjects,and were randomly divided into observation group and control group with 45cases each.The control group was treated with conventional treatment,while the observation group was treated with noninvasive mechanical ventilation on the basis of conventional treatment.Results:The remission rate of dyspnea and the total effective rate of clinical treatment in the observation group were significantly higher than that in the control group,but the length of hospital stay was lower than that in the control group.PO2in the observation group was significantly higher than that in the control group,and PCO2and RR were significantly lower than that in the control group.After 1month follow-up,SF-36life treatment score in the observation group was significantly higher than that in the control group,with significant difference between groups(P<0.05).Conclusion:Noninvasive mechanical ventilation in the treatment of ALHF with respiratory failure in the elderly is helpful to relieve the symptoms of dyspnea,improve blood gas index,and improve the clinical treatment effect and quality of life.
