老年住院带管患者全程智能化管道风险预控管理

Whole process of intelligent pipeline risk pre-control management of elderly hospitalized patients with tubes

目的通过对老年住院带管患者实施全程智能化管道风险预控管理,降低老年住院带管患者的风险,减少护理不良事件的发生。方法以风险预控管理理论为依据,构建依托信息系统的老年住院带管患者管道风险预控管理体系,通过置管前、中、后风险的评估、预控措施的实施及预控效果的评价进行全程智能化管道风险预控管理。结果实施全程智能化管道风险预控管理后引流管无效能发生率由0.46‰下降至0.12‰,非计划拔管率由0.62‰下降至0.20‰,24 h管道高危因素评估率由69.43%上升至88.11%;护士对管道知识的知晓率由81.67%上升至95.00%;患者对管道护理的满意度由86.26%上升至97.62%,差异有统计学意义(P<0.01或P<0.05)。结论老年住院带管患者的全程智能化管道风险预控管理,将管道风险事件的事后处理变为事前预防,能够降低不良事件的发生率,提高护士的临床工作能力,保障老年住院带管患者的安全。

Objective Through the whole process intelligent tube risk pre-control management of elderly hospitalized patients with tube,to reduce the risk of tube and the occurrence of adverse nursing events.Methods Based on the theory of risk pre-control management,using information system to construct the risk precontrol management system for elderly patients with tube.The whole process intelligent tube risk precontrol management was carried out by evaluating the pre,middle and post catheter risk,carrying out the pre-control measures and the effect of pre-control.Results After applying the whole process intelligent tube risk pre-control management,the ineffective rate of drainage tube decreased from 0.46‰ to 0.12‰ and the non-planned tube-pulling rate decreased from 0.62‰ to 0.20‰.The rate of assessment of high risk for 24 hours increased from 69.43% to 88.11% and nurses’ knowledge of tube knowledge increased from 81.67% to 95.00%.Patients’ satisfaction with pipeline nursing increased from 86.26% to 97.62%.The difference were statistically significant(P<0.01 orP<0.05).Conclusion The whole process of intelligent tube risk precontrol management with elderly tube patients can change the post processing of tube risk events into precautionary precaution,reduce the incidence of adverse events,and improve nurses’ clinical ability and guarantee the safety of tube.
