国内外药学查房现状及补偿机制探讨

目的:了解国内外药学查房的开展现状、补偿情况及国外药学查房及收费经验,为国内更好地开展这项工作提供借鉴,实现药学服务的高质量发展。方法:于2019年2月挑选国内药学服务开展具有代表性和先进经验的地区,选择若干所二级和三级医院发放电子问卷,调查内容包括现阶段医院药学查房的开展情况、人员任职资格、补偿情况等;同时,结合文献研究国内外药学查房开展现状及补偿情况。结果:药学查房是专业药学工作中开展广泛的一项,在全国范围内平均开展率为68.5%,属于人员资质系数要求较高的药学服务。在调研的143所医院中,均未对药学查房服务进行收费。国外药学查房工作开展较为成熟规范,虽无明确单独设置药学查房收费项目,但均对开展药学查房所涵盖的相关住院药学服务工作通过收取药事服务费等不同方式进行补偿。结论:应进一步规范药学查房工作内容和相关人员资质要求,加强药师专业素质培养,提高服务水平。同时,探索实践适合国内药学查房相关工作的合理补偿途径,从而体现药师劳动价值,开展更高质量的药学查房工作,保证患者用药安全、有效、经济、合理。

Objective: To understand the current situation and compensation of pharmacy rounds at home and abroad, as well as foreign pharmacy rounds and charging experience, to provide a reference for the country to better carry out this work and achieve high-quality development of pharmacy services. Methods: In February 2019, areas with representative and advanced experience in domestic pharmaceutical service development were selected, and several Grade II and Grade III hospitals were selected to issue electronic questionnaires. The survey content includes the current status of pharmaceutical rounds in hospitals and staff qualifications. Compensation situation, etc .;meanwhile, the current situation and compensation situation of pharmaceutical rounds in China and abroad are studied in combination with the literature. Results: Pharmacy rounds are widely carried out in professional pharmacy work, with an average nationwide development rate of 68.5%, which is a pharmacy service with a high staff qualification coefficient. None of the 143 hospitals surveyed charged for ward rounds. The development of pharmaceutical rounds in foreign countries is relatively mature and standardized. Although there are no separate charges for separate rounds of pharmacy rounds, they all compensate for the related inpatient pharmacy services covered by pharmaceutical rounds by collecting pharmacy service fees. Conclusion: The contents of pharmacy rounds and qualification requirements of related personnel should be further standardized, the professional quality training of pharmacists should be strengthened, and the service level should be improved. At the same time, we will explore and implement reasonable compensation methods suitable for domestic pharmacy rounds, so as to reflect the labor value of pharmacists, carry out higher quality pharmacy rounds, and ensure that patients' medication is safe, effective, economical and reasonable.
