Analysis of influencing factors of hospitalization expenses for patients with colorectal cancer surgery

目的:分析结直肠癌手术患者的一般情况、住院费用及其影响因素,为合理分配使用卫生资源,控制结直肠癌手术患者住院费用及直接经济负担,加强肿瘤专科医院的医疗费用管理提供指导依据。方法:应用SAS 9.4统计软件对数据进行处理和统计分析,进行研究指标的统计描述、单因素统计描述分析、逐步回归方法进行多因素分析。结果:本研究共纳入分析结直肠癌手术患者2417例,平均住院费用74713.11元,主要以材料费、药费为主,分别占总费用的48.35%和29.98%。影响住院费用因素包括:年龄、医疗付费方式、是否结直肠多发恶性肿瘤、是否有肝继发恶性肿瘤、是否伴有慢性病、是否进行非计划再次手术、手术方式、住院天数等。结论:提高结直肠癌的防治水平,深化医药卫生体制改革,提高医务人员的劳务价值,加强医用耗材的监管,加强医院精细化管理,在保证医疗质量的同时提高工作效率,合理控制医疗费用。

Objective: To analyze the general situation of patients with colorectal cancer surgery, hospitalization costs and influencing factors, to rationally allocate and use health resources, to control the hospitalization costs and direct economic burden of colorectal cancer surgery patients, and to strengthen the management of medical expenses in oncology hospitals Provide guidance. Methods: SAS9.4 statistical software was used for data processing and statistical analysis, and statistical description of research indicators, single-factor statistical description analysis, and stepwise regression method were used for multi-factor analysis. Results: A total of 2 417 patients with colorectal cancer surgery were included in this study. The average hospitalization cost was 74 713.11 yuan, which was mainly the cost of materials and medicine, accounting for 48.35% and 29.98% of the total cost. Factors influencing hospitalization expenses include age, medical payment methods, whether multiple colorectal malignancies, whether there are secondary liver malignancies, whether accompanied by chronic diseases, whether unplanned reoperations, surgical methods, length of hospital stay, and so on. Conclusion: Improve the prevention and treatment of colorectal cancer, deepen the reform of the medical and health system, increase the labor value of medical staff, strengthen the supervision of medical consumables, strengthen the fine management of hospitals, improve work efficiency while ensuring medical quality, and rationally control medical expenses.
