摘要
目的探讨增强T2^*加权血管成像(ESWAN)序列多定量参数鉴别子宫肉瘤(US)与子宫肌瘤退变(DH)的价值。资料与方法回顾性分析经手术病理证实的17例US及33例DH患者的临床资料。所有患者术前均行1.5T MRI检查(含ESWAN序列)。由2位观察者分别测量两组患者ESWAN序列后处理参数,包括幅度、相位、R2^*及T2^*值;比较两组患者的参数值,并评估其鉴别诊断效能。结果US组患者相位值为[0.025(0.012,0.056)弧度]、T2^*为[96.95(82.14,113.23)ms],均大于DH组的0.007(-0.002,0.016)弧度、48.15(40.19,56.80)ms,R2^*值小于DH组[(14.04±3.99)Hz比(22.45±5.47)Hz],差异均有统计学意义(P<0.05);组间幅度值差异无统计学意义(P>0.05)。相位、R2^*及T2^*值诊断US的ROC曲线下面积分别为0.854、0.900、0.961。结论ESWAN序列的相位值、R2^*值、T2^*值可有效鉴别US与DH,具有一定的临床应用价值。
Purpose To evaluate the diagnostic efficacy of T2^*enhanced susceptibility-weighted angiography(ESWAN)sequence with multiple quantitative parameters in identifying uterine sarcoma(US)and degenerative hysteromyoma(DH).Materials and Methods The clinical data of 17 patients with US and 33 patients with DH diagnosed by operation and pathology were analyzed retrospectively.All patients underwent preoperative 1.5T MRI(including ESWAN sequence).The parameters including amplitude,phase,R2^*and T2 values were processed after ESWAN sequence in the two groups was measured respectively by two researchers,and the diagnostic efficacy was evaluated by comparing the parameters.Results The phase[0.025(0.012,0.056)radian],T2^*[96.95(82.14,113.23)ms]in US group were significantly bigger than those in DH group[0.007(-0.002,0.016)radian and 48.15(40.19,56.80)ms],but the value of R2^*was smaller than that in DH group[(14.04±3.99)Hz vs.(22.45±5.47)Hz],with statistical significance(P<0.05).There was no statistical significance in difference in amplitude value between the two groups(P>0.05).The areas under the ROC curve of phase,R2^*and T2^*in US group were 0.854,0.900 and 0.961,respectively.Conclusion The phase value,R2^*and T2^*values of ESWAN sequence can effectively identify US and DH,with clinical application value.
