临床护理路径对双相情感障碍躁狂发作患者症状和服药依从性的影响

Effect of clinical nursing pathway on psychiatric symptoms and medication adherence of patients with bipolar mania

目的观察临床护理路径对双相情感障碍躁狂发作患者症状和服药依从性的影响。方法选取2018年6月-12月住院治疗的80例双相情感障碍躁狂发作患者为研究对象,采用随机数字表法分为观察组和对照组,各40例。治疗期间,对照组接受常规护理干预,观察组在对照组基础上实施临床护理路径干预。于患者入院、出院时采用护士用住院患者观察量表(NOSIE)、倍克-拉范森躁狂量表(BRMS)进行精神症状评估,同时统计2组患者的服药依从性。结果出院时,观察组NOSIE总分、总积极因素分数均高于对照组,总消极因素均低于对照组,差异均有统计学意义(P<0.01);观察组BRMS总分均低于对照组,差异有统计学意义(P<0.01);观察组服药依从性均高于对照组,差异有统计学意义(P<0.05)。结论实施临床护理路径有利于改善双相情感障碍躁狂发作住院患者的症状,提高其服药依从性。

Objective To observe the effect of clinical nursing pathway on psychiatric symptoms and medication adherence of patients with patients with bipolar mania. Methods Totally 80 patients with patients with bipolar mania were randomly divided into the observation group and the control group, with 40 cases in each group. All patients received routine care during the treatment period, and those in the observation group were given interventions based on the clinical nursing pathway. The Nurses’ Observation Scale for In-patient Evaluation(NOSIE) and Bech-Rafaelsen Mania Rating Scale(BRMS) were used to evaluate the psychiatric symptoms of patients, and the medication adherence of patients was compared between two groups. Results Patients in the observation group achieved higher score of NOSIE and score of total positive factors compare with those in the control group before discharge form hospital(P<0.01). The scores of total negative factors and BRMS in the observation group was lower than those in the control group(P<0.01). The medication adherence in the observation group was higher than that in the control group(P<0.05). Conclusion The implementation of clinical nursing pathway can improve the psychiatric symptoms and medication adherence of patients with bipolar mania.
