自拟清脑舒络汤治疗中风恢复期患者的疗效及对血液流变学的影响分析

Analysis of the efficacy of self-made Qingnao Shuluo decoction in treating rehabilitation of stroke patients and its influence on hemorheology

目的探讨自拟清脑舒络汤治疗中风恢复期患者的临床效果及对血液流变学的影响。方法 128例中风恢复期患者,采用随机数字表法分为对照组和观察组,每组64例。对照组患者给予阿司匹林肠溶片联合阿托伐他汀钙片治疗,观察组患者在对照组基础上给予自拟清脑舒络汤治疗。比较两组患者的临床疗效、治疗前后血液流变学指标[全血粘度(ηb)、血浆粘度(ηp)、红细胞沉降率(ESR)、红细胞比容(Hct)、纤维蛋白原(FIB)]变化情况及不良反应发生情况。结果观察组患者治疗总有效率为90.63%,高于对照组的76.56%,差异有统计学意义(P<0.05)。治疗后,两组患者ηb、ηp、ESR、Hct、FIB水平均较本组治疗前明显下降,观察组下降程度大于对照组,差异具有统计学意义(P<0.05)。观察组不良反应发生率为9.38%,与对照组的12.50%比较差异无统计学意义(P>0.05)。结论中风恢复期患者应用自拟清脑舒络汤治疗能有效改善患者神经功能,调节血液流变状态,疗效显著,安全可靠,值得临床推广。

Objective To discuss the clinical effect of self-made Qingnao Shuluo decoction in treating rehabilitation of stroke patients and its influence on hemorheology. Methods A total of 128 stroke patients at rehabilitation were divided into control group and observation group by random number table method, with 64 cases in each group. The control group was treated by aspirin enteric coated tablets combined with atorvastatin calcium tablets, and the observation group was treated by self-made Qingnao Shuluo decoction on the basis of the control group. The clinical efficacy, changes of hemorheology indexes [whole blood viscosity(ηb), plasma viscosity(ηP), erythrocyte sedimentation rate(ESR), hematocrit(HCT), fibrinogen(FIB)] and occurrence of adverse reactions was compared between the two groups. Results The total effective rate of treatment was 90.63% in the observation group, which was obviously higher than 76.56% in the control group, and the difference was statistically significant(P<0.05). After treatment, the levels of ηb, ηp, ESR, Hct and FIB in the two groups was obviously lower than those before treatment, and the decline degree in the observation group was greater than that in the control group. Their difference was statistically significant(P<0.05). The incidence of adverse reactions was 9.38% in the observation group, which had no statistically significant difference compared with 12.50% in the control group(P>0.05). Conclusion Application of Qingnao Shuluo decoction shows remarkable efficacy for rehabilitation of stroke patients, and it can effectively improve the neurologic function of patients, regulate the state of hemorheology. It is safe and reliable, and worthy of clinical promotion.
