腹腔镜辅助阴式全子宫切除术在巨大子宫肌瘤治疗中的应用价值

The value of laparoscopic assisted vaginal hysterectomy in the treatment of giant hysteromyoma

目的分析腹腔镜辅助阴式全子宫切除术在巨大子宫肌瘤治疗中的应用价值。方法 86例巨大子宫肌瘤患者,根据入院时间分为研究组与对照组,每组43例。对照组患者采用传统开腹全子宫切除术治疗,研究组患者采用腹腔镜辅助阴式全子宫切除术治疗。比较两组患者手术时间、出血量、排气时间、下床活动时间、住院天数、并发症发生情况、治疗效果、治疗满意度。结果研究组患者手术时间(84.63±11.25)min、出血量(118.47±37.82)ml、排气时间(23.54±9.97)h、下床活动时间(1.74±0.52)d、住院天数(5.27±1.10)d均优于对照组的(125.63±12.32)min、(145.20±44.51)ml、(47.20±12.60)h、(3.98±0.52)d、(7.80±1.50)d,差异均具有统计学意义(P<0.05)。研究组患者并发症发生率4.65%显著低于对照组的23.26%,差异具有统计学意义(P<0.05)。研究组患者的治疗总有效率97.67%高于对照组的72.09%,差异具有统计学意义(P<0.05)。研究组患者的治疗总满意度为97.67%,高于对照组的67.44%,差异具有统计学意义(P<0.05)。结论腹腔镜辅助阴式全子宫切除术对巨大子宫肌瘤患者有明显的治疗效果,缩短住院时间、提高治疗效果且并发症少、下床活动时间早,治疗方法安全,亟待在临床治疗工作中应用。

Objective To analyze the application value of laparoscopic assisted vaginal hysterectomy in the treatment of giant hysteromyoma. Methods A total of 86 giant hysteromyoma patients were divided into research group and control group by admission order, with 43 cases in each group. The control group received traditional open hysterectomy, and research group received laparoscopic assisted vaginal hysterectomy. The operation time, amount of hemorrhage, exhaust time, activity time, hospitalization time, occurrence of complications, treatment effect and treatment satisfaction degree was compared between the two groups. Results The operation time(84.63±11.25) min, amount of hemorrhage(118.47±37.82) ml, exhaust time(23.54±9.97) h, off-bed activity time(1.74±0.52) d and hospitalization time(5.27±1.10) d in the research group was better than(125.63±12.32) min,(145.20±44.51) ml,(47.20±12.60) h,(3.98±0.52) d and(7.80±1.50) d in the control group, and their difference was statistically significant(P<0.05). The incidence of complications 4.65% in the research group was significantly lower than 23.26% in the control group, and the difference was statistically significant(P<0.05). The total effective rate of treatment 97.67% in the research group was higher than 72.09% in the control group, and the difference was statistically significant(P<0.05). The total satisfaction degree of treatment was 97.67% in the research group, which was higher than 67.44% in the control group, and the difference was statistically significant(P<0.05). Conclusion Laparoscopic assisted vaginal hysterectomy shows obvious therapeutic effect in the treatment of patients with giant hysteromyoma, and it can shorten hospitalization time, improve therapeutic effect with less complications, earlier off-bed activity time. It is safe and is worthy of application in clinical work.
