Mini-CEX评估在中医内科临床思维实训考核中的应用

The Application of Mini-CEX Assessment in Clinical Tideation Training Assessment of Internal Medicine of Traditional Chinese Medicine

目的Mini-CEX评估(Mini-Clinical Evaluation Exercise迷你临床演练评估)目前在临床各科实践技能考评中得到有效推广,本文就其在中医内科学临床思维能力和中医经典理论的带教等方面的应用进行探讨,为高素质的中医内科人才培养,为提高住院医师规范化培养的效果,为临床考核提供有效的检验手段。方法在思维实训室,通过实例或者典型患者,让规培医师临床规范诊疗,从病史采集、体格检查、临床诊断、处方用药、沟通技巧、专业态度、中医经典、整体临床胜任力等8个方面,以9分制为计分标准进行判断。结果经过临床1个月的带教,学生在“病史采集、体格检查、中医经典、沟通技巧”三方面有明显的提高,但临床诊断、处方用药、整体临床胜任力等需要进一步加强。结论将Mini-CEX引入中医内科教育,有效带动了专业实习和教育方式的改革,使教学手段多样化,同时为考核提供了有效的方法,临床考评更加客观化。

Objective The evaluation of Mini-CEX(Mini-Clinical Evaluation Exercise Mini CEX)has been effectively promoted in clinical practice skills evaluation.This paper explores the ability of clinical thinking of internal medicine of traditional Chinese medicine and the application of the theory of TCM in teaching,in order to train high-quality talents of internal medicine of traditional Chinese medicine,improve the effect of standardized residency training,provide an effective test method for clinical examination.Methods In the thinking training room,letting the standardized residency making a diagnosis and giving treatment using the example of typical patients or typical physicians.Through 8 aspects of history,physical examination,clinical diagnosis,prescription,communication skills,professional attitude,classics of Chinese medicine and the overall clinical competence,9 points was used for the scoring standard to make a judgment.Results After a month of clinical teaching,the student has significantly improved in history collection,physical examination,classical Chinese medicine and communication skills.But still need to be further strengthened in the clinical diagnosis,prescription and overall clinical competence.Conclusion The introduction of The Mini-CEX in internal medicine of traditional Chinese medicine,promoting the reform of professional training and educational mode effectively,making the teaching methods diversify,providing more effective methods for clinical assessment,making the clinical evaluation more objectively.
