摘要
目的探讨血清25-程维生素D[25—hydroxy vitamin D,25-(OH)D]、超敏C-反应蛋白(hypersensitive C—reactive protein,hs-CRP)、血清降钙素原(procalcitonin,PCT)与新生儿重症肺炎的关系。方法以深圳市宝安区石岩人民医院2015年2月一2019年2月116例细菌感染性新生儿肺炎患儿为观察组,依据患儿病情轻重分为重症肺炎组46例,轻症肺炎组70例;选取同期在本院出生的体检新生儿70例设为对照组;对比3组血清25-(OH)D、hs-CRP、PCT水平,并分析其与新生儿肺炎严重程度的相关性及诊断特异性。结果重症肺炎组(7.3±3.1 ng/mL)血清25-(OH)明显低于轻症肺炎(11.0±4.9 ng/mL)组及对照组(17.6±2.8 ng/mL),且轻症肺炎组明显低于对照组(均P<0.05);重症肺炎组(23.8±4.2 mg/L,1.56±0.34 ng/mL)血清hs-CRP、PCT明显高于轻症肺炎(12.9±1.8 mg/L,0.73±0.15 ng/mL)组及对照组(2.2±0.9 mg/L,0.06±0.02 ng/mL),且轻症肺炎组明显高于对照组(均P<0.05)。血清25-(OH)D水平与患儿肺炎严重程度呈负相关(r=-0.52,P=0.003),血清hs-CRP、PCT与患儿肺炎严重程度呈正相关(r=0.436,P=0.012;r=0.452,P=0.002)。血清25-(OH)D、hs-CRP、PCT联合检测对重症肺炎诊断价值明显高于单独检测。结论血清25-(OH)D、hs-CRP、PCT与新生儿肺炎严重程度具有密切关系,可用于辅助诊断重症肺炎及患儿病情的评估。
Objective To investigate the relationship between serum 25-(OH)D,hs-CRP,PCT and neonatal severe pneumonia.Methods In our hospital from February 2015 to February 2019,116 children with bacterial infectivity neonatal pneumonia were selected as the observation group.According to the severity of the disease,46 patients were diagnosed as severe pneumonia and 70 patients with mild pneumonia.The same period was born in our hospital.Seventy patients with physical examination were enrolled in the control group.The serum levels of 25-(OH)D,hs-CRP and PCT were compared between the three groups,and their correlation with the severity of neonatal pneumonia and the diagnostic specificity were analyzed.Results Serum 25-(OH)in the severe pneumonia group(7.3±3」ng/mL)was significantly lower than that in the mild pneumonia(11.0±4.9 ng/mL)group and the control group(17.6±2.8 ng/mL),and the mild pneumonia group was obvious.Lower than the control group(P<0.05);Serum hs-CRP and PCT in the severe pneumonia group(23.8±4.2 mg/L,1.56±0.34 ng/mL)were significantly higher than mild pneumonia(12.9±1.8 mg/L,0.73±0.15 ng/mL)group and control group(2.2±0.9 mg/L,O.O6±O.O2 ng/mL),and the mild pneumonia group was significantly higher than the control group(P<0.05).Serum 25-(OH)D level was negatively correlated with the severity of pneumonia in children(r=-0.52,P=0.003).Serum hs-CRP and PCT were positively correlated with the severity of pneumonia in children(r=0.436,P=0.012);(r=0.452,P=0.002).The combined detection of serum 25-(OH)D,hs-CRP and PCT was significantly higher in the diagnosis of severe pneumonia than in the separate test.Conclusion Serum 25-(OH)D,hs-CRP and PCT are closely related to the severity of neonatal pneumonia and can be used to assist in the diagnosis of severe pneumonia and the condition of children.
