二维斑点追踪显像技术评价左心室构型正常高血压患者右心室功能的效果分析 被引量:4

Evaluation of Right Ventricular Function in Hypertension Patients with Normal Left Ventricular Configuration by Two-dimensional Speckle Tracking Imaging

摘要 目的探讨应用二维斑点追踪显像技术评价左心室构型正常高血压患者右心室功能的效果。方法选取2017年1月-2018年7月在朝阳市中心医院就诊或住院的左心室构型正常的高血压患者30例为观察组,选择同期进行健康体检的非高血压者30例为对照组,观察并比较两组常规超声心动图、右心组织多普勒血流成像及二维斑点追踪显像检査结果。结果常规超声心动图检查:两组左心房内径、二尖瓣口舒张晚期峰值流速以及二尖瓣口舒张早期峰值流速/舒张晚期峰值流速值,差异均有统计学意义(P<0.05)。组织多普勒成像检査:两组三尖瓣口舒张早期峰值速度、舒张晚期峰值速度以及三尖瓣口舒张早期峰值速度与舒张晚期峰值速度比值、减速时间、等容舒张时间,差异均有统计学意义(P<0.05)。二维斑点追踪显像检查:两组右心室游离壁各节段心肌收缩期峰值应变、心肌应变率、舒张早期峰值应变率以及舒张早期峰值应变率与舒张晚期峰值应变率比值,差异均有统计学意义(P<0.05)。两组其他各项影像学检查指标差异均无统计学意义(P>0.05)。结论对于无左心室构型改变的高血压患者,右心功能已有不同程度改变,二维斑点追踪显像技术有希望成为定量评价右心功能的一种新方法。 Objective To evaluate the effect of two-dimensional speckle tracking imaging(STI)in examining right ventricular function in hypertensive patients with normal left ventricular configuration.Methods A total of 30 hypertensive patients with normal left ventricular configuration(observation group)and 30 healthy volunteers(control group)were selected from January 2017 to July 2018 in Chaoyang Central Hospital.The results of routine echocardiography,doppler imaging of right ventricular tissue and two-dimensional STI were observed and compared between the two groups.Results Routine echocardiography:there were significant differences in left atrial diameter,late diastolic peak velocity of mitral valve and the ratio of early diastolic peak velocity to late diastolic peak velocity between the two groups(P<0.05).Tissue Doppler imaging:there were significant differences in peak velocity of tricuspid valve in early diastole,late diastolic,ratio of early diastolic peak velocity to late diastolic peak velocity,deceleration time and isovolumic diastolic time between the two groups(P<0.05).Two-dimensional spot tracking imaging:there were statistically significant diflerenccs in systolic peak strain of right ventricular free wall,myocardial strain rate,early diastolic peak strain rate and ratio of early diastolic peak strain rate to late diastolic peak strain rate between the two groups(P<0.05).Conclusion In hypertensive patients without left ventricular con figuration changes,right ventricular fimetion has been changed in varying degrees.STI is expected to be a new method for quantitative evaluation of right ventricular function.
