奥卡西平对癫痫部分性发作患儿脑电图、认知功能和氧化应激的影响 被引量:8

Effects of oxcarbazepine on electroencephalogram,cognitive function and oxidative stress in children with partial seizures of epilepsy

摘要 目的分析奥卡西平对癫痫部分性发作患儿脑电图、认知功能和氧化应激的影响。方法选取河北医科大学第二医院2017年1月至2019年1月收治的116例癫痫部分性发作患儿,采用随机数表法将其分为奥卡西平组(58例)和丙戊酸钠组(58例),分别给予患儿奥卡西平和丙戊酸钠口服治疗。比较两组患儿脑电图、认知功能、氧化应激等指标变化,分析两种治疗方案的效果与安全性。结果两组患儿脑电图痫样放电消失和减少≥50%的比例比较无统计学意义(均P>0.05)。治疗6个月后,两组患儿蒙特利尔认知评估量表评分与治疗前比较均无统计学意义(均P>0.05),且两组患儿比较亦无统计学意义(P>0.05)。治疗6个月后,奥卡西平组患儿一氧化氮、黄嘌呤氧化酶和丙二醛水平均显著低于丙戊酸钠组(均P<0.05)。两组患儿治疗总有效率比较无统计学意义(P>0.05),但奥卡西平组患儿癫痫控制率显著高于丙戊酸钠组(P<0.05)。两组患儿不良反应发生率比较无统计学意义(P>0.05)。结论奥卡西平与丙戊酸钠对癫痫部分性发作患儿脑电图的改善效果相当,对认知功能无明显影响,但奥卡西平能够通过改善患儿氧化应激,实现更为理想的癫痫控制率。 Objective To analyze the effects of oxcarbazepine on electroencephalogram,cognitive function and oxidative stress in children with partial seizures of epilepsy.Method 116 children with partial seizures of epilepsy admitted to the Second Hospital of Hebei Medical University from January 2017 to January 2019 were divided into oxcarbazepine group and valproic acid group by random number table method,58 cases in each group,and they were given oxcarbazepine and valproic acid oral treatment respectively.The changes of electroencephalogram,cognitive function and oxidative stress were compared between the two groups,and the effect and safety of the two treatment schemes were analyzed.Result There was no significant difference in the proportion of epileptiform discharges disappeared or decreased by more than 50%in electroencephalogram between the two groups(all P<0.05).After 6 months of treatment,the scores of the Montreal cognitive assessment scale of the two groups of children were not statistically significant compared with those before treatment(all P>0.05),and the comparison between the two groups of children was not statistically significant(P>0.05).After 6 months of treatment,the levels of nitric oxide,xanthine oxidase and malondialdehyde in oxcarbazepine group were significantly lower than those of valproic acid group(all P<0.05).There was no significant difference in the total effective rate between the two groups(P>0.05),but epilepsy control rate of oxcarbazepine group was significantly higher than that of valproic acid group(P<0.05).The incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusion Oxcarbazepine and valproic acid can improve electroencephalogram of children with partial seizures of epilepsy,but have no significant effect on cognitive function.Oxcarbazepine can improve oxidative stress of children and achieve better control rate of epilepsy.
