类风湿关节炎合并骨质疏松症伴躯体疼痛和功能受限患者状况分析

Analysis of the status of patients with rheumatoid arthritis and osteoporosis with body pain and limited function

目的探讨类风湿关节炎(rheumatoid arthritis,RA)合并骨质疏松症(osteoporosis,OP)伴躯体疼痛和功能受限患者的状况。方法选取2017年6月至2019年6月绵阳市中心医院收治的137例RA合并OP患者为研究对象,其中伴躯体疼痛和功能受限者36例。采用焦虑自评量表(self rating anxiety scale,SAS)和抑郁自评量表(self rating depression scale,SDS)评价患者情绪状况;采用简易McGill疼痛量表(simple McGill pain scale,SF-MPQ)评价患者疼痛情况,包括疼痛评定指数(pain rating index,PRI)和现有疼痛强度(present pain intensity,PPI);采用功能大体评定量表(global assessment function scale,GAF)评价患者社会功能;采用健康状态调查问卷(the short form-36 health survey,SF-36)评价患者生活质量。比较伴或不伴躯体疼痛和功能受限患者SAS和SDS评分、PRI和PPI评分、GAF评分及SF-36量表评分。结果躯体疼痛和功能受限患者SAS、SDS、PRI和PPI评分均显著高于无躯体疼痛和功能受限者(均P<0.05);躯体疼痛和功能受限患者GAF量表和SF-36各量表评分均显著低于无躯体疼痛和功能受限(均P<0.05)。结论RA合并OP伴躯体疼痛和功能受限患者存在焦虑和抑郁情绪,且患者疼痛明显,社会功能和生活质量下降。

Objective To investigate the status of patients with rheumatoid arthritis(RA)and osteoporosis(OP)with body pain and limited function.Method From June 2017 to June 2019,137 RA patients with OP admitted to Mianyang Central Hospital were selected as the study subjects,including 36 patients with physical pain and limited function.Self rating anxiety scale(SAS)and self rating depression scale(SDS)were used to evaluate the emotional state of patients,simple McGill pain scale(SF-MPQ)was used to evaluate the patients'pain,including pain rating index(PRI)and present pain intensity(PPI).Global assessment function scale(GAF)was used to evaluate the patients'social function.The short form-36 health survey(SF-36)was used to evaluate the quality of life of patients.SAS,SDS,PRI,PPI,GAF and SF-36 scale scores were compared in patients with or without physical pain and functional limitation.Result SAS,SDS,PRI and PPI scores of patients with physical pain and functional restriction were significantly higher than those without physical pain and functional restriction(all P<0.05).GAF and SF-36 scores of patients with physical pain and functional restriction were significantly lower than those without physical pain and functional restriction(all P<0.05).Conclusion There are anxiety and depression in RA patients with OP accompanied by physical pain and limited function,and the pain,social function and quality of life of RA patients are obviously decreased.
