摘要
目的采用Meta分析评估半乳糖凝集素3对心力衰竭(心衰)的诊断价值。方法检索PubMed、EMbase、万方数据库和中国知网自数据库建库开始至2018年11月30日之前收录的关于半乳糖凝集素3对心力衰竭(心衰)诊断效能的中英文文献并提取数据,使用诊断准确性研究的质量评价工具-2(QUADAS-2)对纳入文献进行质量评估,采用MetaDiSc1.4软件全面分析半乳糖凝集素3诊断心力衰竭的价值。结果共纳入文献14篇,纳入研究存在非阈值效应引起的异质性,采用随机效应模型合并结果,合并灵敏度为0.78(95%CI:0.76~0.80);合并特异度为0.65(95%CI:0.63~0.68);合并阳性似然比为2.85(95%CI:2.07~3.92);合并阴性似然比0.28(95%CI:0.21~0.39);合并诊断比值比(DOR)为12.48(95%CI:6.48~24.06);ROC曲线下面积(AUC)为0.8531。结论半乳糖凝集素3对心衰有中等偏高程度的诊断价值,但仍需要设计合理的大样本研究来证明。
Objective To review the diagnostic value of galectin-3(Gal-3)to heart failure(HF)by applying Meta-analysis.Methods The databases of PubMed,EMbase,WanFang Data and CNKI were retrieved for searching the Chinese and English documents about the diagnostic efficacy of Gal-3 to HF from database establishing time to Nov.30,2018,and data was extracted.The quality of included documents was reviewed by using quality assessment of diagnostic accuracy studies-2(QUADAS-2)for study the diagnostic accuracy.The diagnostic value of Gal-3 to HF was comprehensively analyzed by using MetaDiSc1.4 software.Results There were totally 14 documents included,which had the heterogeneity caused by non-threshold effect.A randomized effect model was used to combine results of diagnostic value of Gal-3 to HF,and the results showed that the combined sensitivity was 0.78(95%CI:0.76~0.80),combined specificity was 0.65(95%CI:0.63~0.68),combined positive likelihood ratio was 2.85(95%CI:2.07~3.92),combined negative likelihood ratio was 0.28(95%CI:0.21~0.39),combined diagnostic odds ratio(DOR)was 12.48(95%CI:6.48-24.06),and AUC under ROC was 0.8531.Conclusion Gal-3 has approximately higher diagnostic value to HF,while the conclusion is needed to be certified by the study with large sample and reasonable design.
