基于合理信念替代疗法的心理护理干预对甲状腺相关眼病患者的影响

Effects of Psychological Nursing Intervention Based on Reasonable Belief Substitution Therapy on Patients with Thyroid Associated Ophthalmopathy

目的:探讨基于合理信念替代疗法的心理护理干预对甲状腺相关眼病(TAO)患者心理状态、生活方式和治疗依从性的影响。方法:选择2017年2月1日~2019年1月31日就诊的74例TAO患者作为研究对象,采用随机数字表法将其分为对照组和观察组各37例,对照组给予常规护理干预,观察组在常规护理干预基础上给予基于合理信念替代疗法的心理护理干预;比较两组不同时间焦虑程度[采用焦虑自评量表(SAS)]、抑郁指数,治疗期间依从性,干预前后健康生活方式认知[采用促进生活方式量表Ⅱ(HPLPⅡ)]。结果:出院时、出院1个月、治疗结束时,观察组SAS评分和抑郁指数均低于对照组(P<0.05);观察组治疗依从性高于对照组(P<0.05);干预后,观察组营养、人际关系、健康责任性、自我实现、运动锻炼、压力管理得分均高于对照组(P<0.05)。结论:基于合理信念替代疗法的心理护理干预可缓解TAO患者的不良心理情绪,提高治疗依从性,促进健康生活方式养成,有利于控制病情进展,值得临床推广应用。

Objective:To investigate the effect of psychological nursing intervention based on reasonable belief substitution therapy on the psychological state,lifestyle and treatment compliance of patients with thyroid associated ophthalmopathy(TAO).Methods:A total of 74 patients with TAO who visited the clinic from February 1,2017 to January 31,2019 were selected as the study subjects.They were divided into the control group and the observation group with 37 cases each by the random number table method.Different time anxiety(by the SAS),depression index,compliance during treatment,and healthy way of life before and after the intervention cognitive(by HPLPⅡ)were compared between the two groups.Results:SAS score and depression index in the observation group were lower than those in the control group at discharge,1 month after discharge,and at the end of treatment(P<0.05).The treatment compliance of the observation group was higher than that of the control group(P<0.05).After intervention,the scores of nutrition,interpersonal relationship,health responsibility,self-actualization,exercise and stress management in the observation group were all higher than those in the control group(P<0.05).Conclusion:Psychological nursing intervention based on reasonable belief substitution therapy can alleviate the adverse psychological mood of TAO patients,improve treatment compliance,promote healthy lifestyle development,and help control the progress of the disease,which is worthy of clinical promotion and application.
