卡维地洛与普萘洛尔对肝硬化并发食管静脉曲张破裂出血预防作用及对静脉血流动力学影响

Effect of Entecavir Combined with Dahuang Zhechong Pill on Liver Function,Virological Indexes and Fibrosis Markers in Patients with Compensated Chronic Hepatitis B-related Cirrhosis

目的:探讨肝硬化并发食管静脉曲张的患者应用卡维地洛治疗与普萘洛尔治疗预防破裂出血作用及对静脉血流动力学影响情况。方法:选取肝硬化并发食管静脉曲张的患者80例,采用数字表抽取法随机分组,就采用卡维地洛给药方案治疗(观察组,n=40)与采用普萘洛尔给药方案治疗(对照组,n=40)总出血率、血流动力学指标等展开对比。结果:观察组总出血率低于对照组(P<0.05)。观察组临床治疗总有效率高于对照组(P<0.05)。观察组治疗后静脉血流动力学指标降低或减少程度较对照组更为显著(P<0.05)。观察组药物服用剂量明显少于对照组(P<0.05)。两组药物不良反应率经对比无明显差异(P>0.05)。结论:肝硬化并发食管静脉曲张患者应用卡维地洛给药方案治疗,相较普萘洛尔,可有效防范破裂出血事件率,提高总有效率,且利于保持静脉血流动力学稳定,减少药物使用剂量,同时具较高安全性。

Objective:To investigate the effect of entecavir combined with Dahuang Zhechong pill on liver function,virological indexes and fibrosis markers in patients with compensated chronic hepatitis B-related cirrhosis.Methods:Eighty patients with cirrhosis complicated with esophageal varices were randomly divided into two groups by the method of digital table.The total bleeding rate and hemodynamic indexes were compared between carvedilol group(observation group,n=40)and propranolol group(control group,n=40).Results:After treatment,liver function,virological indexes and fibrosis markers decreased significantly in the two groups(P<0.05).In the observation group,ALB、PTA、HBeAg and HBsAg negative-transforming rate were higher than the control group(P<0.05),and AST、ALT、TBIL、Child-Pugh score、HBV-DNA level、HA、IV-C、PCⅢand LN were significantly lower in observation group than the control group(P<0.05).Conclusion:Entecavir combined with Dahuang Zhechong pill can improve the liver function and significantly decrease fibrosis markers and virological indexes was also reduced in patients with compensated chronic hepatitis B-related cirrhosis.So it was worthy of clinical promotion.
