糖尿病足患者血流变及黏附因子表达及意义

摘要 目的探究糖尿病足患者血流变及黏附因子表达和意义,为临床提供参考。方法选取2018年2月-2019年2月期间该院接诊的糖尿病足患者,对糖尿病患者进行评估,依据Wagner评级标准,32例1级患者为观察一组,32例2级患者为观察二组,32例3级患者为观察三组。分别检测三组患者的血流变、血管内皮细胞黏附因子-1、细胞黏附因子-1。并分析黏附因子与血流变各项指标之间的相关性。结果血流变结果显示,一组患者的血小板聚集指数,低切指数,血浆比黏度要远远低于二组患者和三组患者,其中三组患者的血小板聚集指数,低切指数,血浆比黏度情况要显著高于其他两组,差异有统计学意义(P<0.05);通过对患者黏附因子的检测可以发现,三组患者中,随着糖尿病足的加重,患者的血管内皮黏附因子-1和细胞黏附因子-1的含量在逐渐增加。观察三组患者的血管内皮黏附因子-1和细胞黏附因子-1的含量要显著高于对照组,差异有统计学意义(P<0.05);细胞黏附因子-1与血浆比黏度和低切黏度之间存在明显的正相关性,但与高切黏度和血小板凝集指数之间并无较为显著的相关关系。血管内皮黏附因子-1与血浆比黏度、低切黏度、血小板凝集指数之间存在明显的正相关性,但与高切黏度并无明显相关性。结论糖尿病足的发生与黏附因子、血流变密切相关,这两种因素可能促进糖尿病足的形成和发展,在对糖尿病足患者的临床诊断中可重点关注黏附因子、血流变化,为早期确诊提供依据。 Objective To investigate the expression and significance of hemorheology and adhesion factors in patients with diabetic foot,and to provide reference for clinical.Methods Patients with diabetic foot who were admitted to our hospital from February 20 to February 2019 were enrolled.The patients were diagnosed with diabetes.According to the Wagner rating criteria,32 patients with grade 1 were observed and 32 patients with grade 2 were observed.Group,32 patients with grade 3 were observed in three groups.Blood rheology,vascular endothelial cell adhesion factor-1 and cell adhesion factor-1 were detected in three groups.And the correlation between adhesion factors and blood rheology indicators was analyzed.Results The Results of blood rheology showed that the platelet aggregation index,low cut index and plasma specific viscosity of a group of patients were much lower than those of the two groups and three groups of patients,and the platelet aggregation index,low cut index and plasma ratio of the three groups.Viscosity was significantly higher than the other two groups,the difference was statistically significant(P<0.05);through the detection of patient adhesion factor,it can be found that in the three groups of patients,with the increase of diabetic foot,the patient’s vascular endothelial adhesion factor-1 and cell adhesion The content of factor-1 is gradually increasing.The levels of vascular endothelial adhesion factor-1 and cell adhesion factor-1 in the three groups were significantly higher than those in the control group,the difference was statistically significant(P<0.05);cell adhesion factor-1 and plasma specific viscosity and low cut There was a significant positive correlation between the viscosities,but there was no significant correlation between the high viscosities and the platelet aggregation index.There was a significant positive correlation between vascular endothelial adhesion factor-1 and plasma specific viscosity,low cut viscosity,and platelet aggregation index,but no significant correlation with high cut viscosity.Conclusion The occurrence of diabetic foot is closely related to adhesion factor and blood rheology.These two factors may promote the formation and development of diabetic foot.In the clinical diagnosis of diabetic foot patients,focus on adhesion factors and blood rheology changes.Provide a basis for diagnosis.
