脑卒中后恢复期站立时足底压力分布观察 被引量:2

Observation on the Distribution of Plantar Pressure During Standing Period after Recovery from Stroke

摘要 目的研究脑卒中后恢复期站立平衡1级患者站立时足底压力的分布情况。方法选取脑卒中后3~6个月、站立平衡1级、无严重认知功能障碍的恢复期患者,运用FootScan足底压力分布测量装置,在赤足状态下静态站立时的足底压力分布,并通过健患足压力分布、患足前足与后足压力分布、健患足外展角等指标进行分析。结果脑卒中后站立平衡1级患者的,患侧全足底压力、前足压力、后足压力均小于健侧足,差异均有统计学意义(P<0.01);患侧后足底压力约为前足底压力的1倍以上,但小于健足后足底压力占前足底压力的比值,差异有统计学意义(P<0.05)。结论通过脑卒中后恢复期站立平衡1级患者站立时足底压力的检测,我们发现此类患者主要通过健足负重,而且约负体重的50%以上。同时发现,患足的负重比健足更为前移,前足负重比例有所增加。另外我们发现患足的外展角较健足大。这些结果,可以为脑卒中后站立平衡训练、步行功能训练提供治疗依据。 Objective To study the distribution of plantar pressure in patients with grade 1 standing balance after stroke during convalescence. Methods Three to six months after stroke, the convalescent patients with grade 1 standing balance and no severe cognitive impairment were selected. FootScan plantar pressure distribution measuring device was used to measure the plantar pressure distribution during standing in barefoot state, and the pressure distribution of healthy feet,forefoot and hind foot, abduction angle of healthy feet were used. And other indicators were analyzed. Results The total plantar pressure, forefoot pressure and hind foot pressure of the affected side were lower than those of the healthy side after stroke(P<0.01), and the pressure of the affected side was more than twice that of the forefoot, but less than the ratio of the plantar pressure of the healthy side to the plantar pressure of the forefoot, the difference was statistically significant(P<0.01). Significance(P<0.05). Conclusion By measuring the plantar pressure of the patients with grade 1 standing balance during the recovery period after stroke, we found that these patients were mainly able to bear the weight of healthy feet, and about 50% of the body weight. At the same time, it was found that the weight of affected foot moved forward more than that of healthy foot, and the weight-bearing proportion of forefoot increased. In addition, we found that the abduction angle of the affected foot was larger than that of the healthy foot. These results can provide therapeutic basis for standing balance training and walking function training after stroke.
