摘要
目的分析糖尿病足部感染患者病原菌检验与耐药性。方法时间段为2017年1月-2019年7月为止,选取患者为糖尿病足部感染者100例作为该次对象研究,回顾性分析其全部的临床资料,所有患者均检验病原菌,并分析其耐药性。结果病原菌分布:该组100例患者分离出病原菌共132株,有64例分离病原菌≥2株,足部多重感染率为64.00%。132株病原菌中革兰阳性菌90株(68.18%),其中肠球菌22株,金黄色葡萄球菌68株;革兰阴性菌42株(31.82%),其中株铜绿假单胞菌16株,肺炎克雷伯菌14株,大肠埃希菌12株。耐药性:对于各种抗生素,肠球菌的敏感性均较差,差异无统计学意义(P>0.05);对于替考拉宁和万古霉素,金黄色葡萄球菌敏感性均为100.00%,与其他抗生素相比差异有统计学意义(P<0.05);对于亚胺培南、美罗培南,大肠埃希菌敏感性均为100.00%,肺炎克雷伯菌敏感性均为85.70%,铜绿假单胞菌敏感性均为75.00%,且与其他抗生素相比差异有统计学意义(P<0.05)。结论糖尿病足部感染患者可由多种病原菌致病,且病原菌耐药性较强,因此需加强检验各类病原菌,以期为患者临床诊治提供可靠依据。
Objective To analyze the pathogenic bacteria test and drug resistance of diabetic foot infections.Methods From January 2017 to July 2019,100 patients with diabetic foot infections were selected as the subjects for this study.The clinical data were retrospectively analyzed.All patients were tested for pathogens and their drug resistance was analyzed.Results Pathogen distribution:A total of 132 strains of pathogens were isolated from 100 patients in this group,64 cases of pathogens(>2 strains)were isolated,and the foot multiple infection rate was 64.0%.Of the 132 pathogenic bacteria,90 were Gram-positive(68.18%),22 were Enterococcus,68 were Staphylococcus aureus,42 were Gram-negative(31.82%),of which 16 were Pseudomonas aeruginosa,14 were Klebsiella pneumoniae and 12 were Escherichia coli.Drug resistance:The sensitivity of Enterococcus to various antibiotics was poor,the difference was not statistically significant(P>0.05);for teicoplanin and vancomycin,the sensitivity of Staphylococcus aureus was 100%,which was different from other antibiotics,the difference was statistically significant(P<0.05);for imipenem and meropenem,the sensitivity of Escherichia coli was 100%,that of Klebsiella pneumoniae was 85.7%,and that of Pseudomonas aeruginosa was 85.7%.Sensibility was 75.0%,and there was a difference compared with other antibiotics,the difference was statistically significant(P<0.05).Conclusion Diabetic foot infections can be caused by a variety of pathogenic bacteria,and the resistance of pathogenic bacteria is strong.Therefore,it is necessary to strengthen the detection of various pathogenic bacteria in order to provide a reliable basis for clinical diagnosis and treatment of diabetic foot infections.
