miR-125a在支气管哮喘患儿血浆中的表达及其临床意义

摘要 目的评估miR-125a在儿童支气管哮喘中的表达及其与疾病患病风险、急性发作风险、严重程度与炎症水平的关系。方法连续纳入急性发作期、缓解期支气管哮喘患儿和匹配的健康儿童各70例,收集受试者实验室检测及肺通气功能检测结果。采用实时荧光定量聚合酶链式反应检测受试者血浆miR-125a表达水平,采用酶联免疫吸附试验检测受试者血浆中肿瘤坏死因子(TNF-α),白细胞介素(IL-)1β,IL-6和IL-17的水平。结果miR-125a在急性发作期支气管哮喘患儿血浆中表达水平显著低于缓解期支气管哮喘患儿(P<0.001)和健康儿童(P<0.001),在缓解期支气管哮喘患儿血浆中表达水平也显著低于健康儿童(P=0.022)。受试者工作特征曲线表明,miR-125a能显著区分3组受试者。同时,miR-125a表达水平与急性发作期支气管哮喘患儿疾病严重程度(P<0.001)、TNF-α(P=0.005)、IL-1β(P=0.007)及IL-17(P=0.037)呈显著负相关,与第1秒用力呼气容积(FEV1)与用力肺活量(FVC)之比(FEV1/FVC)(P<0.001)和FEV1实测/预测值百分比(P<0.001)呈显著正相关。miR-125a表达水平还与缓解期支气管哮喘患儿TNF-α(P=0.032)及IL-17(P=0.001)表达水平和健康儿童IL-6(P=0.024)及IL-17(P=0.026)表达水平呈显著负相关。结论循环miR-125a表达水平与儿童支气管哮喘患病风险、急性发作风险、严重程度及炎症水平呈显著负相关。 Objective To investigate the association of miR-125a expression with disease risk,exacerbation risk,disease severity and inflammation level in childhood bronchial asthma.Methods 70 asthma exacerbation children,70 asthma remission children and 70 matched healthy children were consecutively recruited in this study.Experimental tests and lung function indexes of participants were recorded.miR-125a expressions in plasma were determined by real-time quantitative polymerase chain reaction;tumor necrosis factor(TNF)-α,interleukin(IL)-1β,IL-6,and IL-17 levels were determined by enzyme-linked immunosorbent sorbent assay.Results miR-125a expression was lower in asthma exacerbation children compared with remission children and healthy children,and it was also lower in remission children compare with healthy children.Receiver operating characteristic curves showed that miR-125a is able to differentiate the three groups.Meanwhile,miR-125a expression was negatively associated with disease severity,TNF-α,IL-1βand IL-17 levels and positively associated with FEV1/FVC and FEV1(actual)/FEV1(predicted)of the three group participants.Besides,miR-125a was also negatively correlated with TNF-αand IL-17 of remission patients and IL-6 as well as IL-17 of healthy children.Conclusion Circulating miR-125a expression correlates with decreased disease risk,exacerbation risk,disease severity and inflammation level in childhood bronchial asthma.
