腹腔镜手术治疗中低位直肠癌的临床疗效及对机体免疫功能的影响

Clinical Effect of Laparoscopic Surgery for Mid-and Low Rectal Carcinoma,and its Influence on Body Immuno-function

为探讨腹腔镜手术治疗中低位直肠癌的临床疗效及对机体免疫功能的影响,将2016年1月至2018年12月收治的96例中低位直肠癌随机分为观察组和对照组,各48例,观察组行腹腔镜直肠癌根治术,对照组行开腹直肠癌根治术,对比2组临床疗效及手术前后免疫指标水平。结果显示,观察组术中出血量、术后肛门排气时间、下床活动时间、住院时间及并发症发生率均明显少于对照组,P<0.05。2组术前外周血T淋巴细胞CD3^+、CD4^+及血清IgA、IgM水平比较差异均无统计学意义,P>0.05;术后2组CD3^+、CD4^+、IgA、IgM水平均降低,但观察组各指标水平明显高于对照组,P<0.05。结果表明,腹腔镜手术治疗中低位直肠癌患者创伤小、术后恢复快、对机体免疫功能影响小,值得临床推广应用。

This study was to investigate the clinical effect of laparoscopic surgery for mid-and low rectal cancer,and its influence on body immuno-function,randomly divided 96 cases with mid-or low rectal cancer(treated during 2016-01-2018-12)into observation group(n=48,received laparoscopic radical procedure for rectal cancer)and control group(n=48,open radical procedure for rectal cancer);then compared both groups’clinical effect and the levels of immuno-norms before and after surgery.As results,in intraoperative bleeding volume,the time for postoperative flatus,the time to ambulate,hospitalization time,and the incidence of complication observation group was significantly less than control group(P<0.05,all);before surgery in the levels of peripheral CD3^+,CD4^+,and serum IgA,IgM there was no statistical difference between the two groups(P>0.05),but after surgery both groups’levels of above every indexes were diclined,and,in observation group the levels still significantly higher than in control group(P<0.05).Results show that laparoscopic surgery for mid-and low rectal cancer has such advantages as less trauma,faster recovery after surgery,and mild influence on body immuno-function.It is worthy of clinical popularization and application.
