局部亚低温治疗对高血压性脑出血患者血清hs-CRP及Hcy水平的影响研究 被引量:17

摘要 目的探讨局部亚低温治疗对高血压性脑出血患者血清hs-CRP及Hcy水平的影响。方法将2016年7月至2018年7月期间我院的40例高血压脑出血患者按照随机数字表法分为对照组与试验组,各为20例。两组患者均给予脱水降颅压、控制血压、调节水电解质、酸碱平衡、防止并发症的发生、脑保护等基本药物治疗。在此基础上,实验组患者给予头颅亚低温治疗。比较两组患者治疗前后简易智力状态检查量表(MMSE)评分、神经功能缺损量表(NIHSS)评分、血清超敏-C反应蛋白(hs-CRP)与同型半胱氨酸(Hcy)水平。结果①对照组患者治疗后MMSE及NIHSS评分较治疗前均显著下降(P均<0.05),试验组患者治疗后MMSE评分较治疗前显著升高(P<0.05),试验组患者治疗后NIHSS评分较治疗前显著下降(P<0.05)。试验组治疗后MMSE评分显著高于对照组治疗后(P<0.05),试验组治疗后NIHSS评分显著低于对照组治疗后(P<0.05);②两组患者治疗后血清hs-CRP及Hcy水平较治疗前均显著下降(P均<0.05),且试验组患者治疗后血清上述2项指标水平均分别显著低于对照组治疗后(P均<0.05)。结论局部亚低温治疗可改善高血压性脑出血患者认知功能及神经功能缺损状况,且可有效降低患者血清hs-CRP及Hcy水平,对治疗高血压脑出血具有一定的临床疗效。 Objective To investigate the effect of local mild hypothermia on serum levels of hs-CRP and Hcy in patients with hypertensive intracerebral hemorrhage.Methods 40 patients with hypertensive intracerebral hemorrhage in our hospital from July 2016 to July 2018 were divided into control group and experimental group according to random number table method,20 cases each.Two groups of patients were given dehydration to reduce intracranial pressure,control blood pressure,regulate water and electrolyte,acid-base balance,prevent complications,brain protection and other basic drugs.On this basis,patients in the observation group were given mild hypothermia treatment.The MiniMental State Examination(MMSE),neurological deficit scale(NIHSS),high-sensitivity C-reactive protein(hs-CRP)and homocysteine(Hcy)levels were compared between the two groups before and after treatment.Results The scores of MMSE and NIHSS in the control group were significantly lower than those before treatment(P<0.05).The scores of MMSE in the experimental group were significantly higher than those before treatment(P<0.05).The scores of NIHSS in the experimental group were significantly lower than those before treatment(P<0.05).After treatment,the MMSE score in the experimental group was significantly higher than that in the control group(P<0.05),and the NIHSS score in the experimental group was significantly lower than that in the control group(P<0.05).Secondly,the serum hs-CRP and Hcy levels in the two groups were significantly lower than those before treatment(P<0.05),and the serum levels of the two indexes in the experimental group were significantly lower than those in the control group(P<0.05).<0.05).Conclusion Local mild hypothermia therapy can improve cognitive function and neurological deficit in patients with hypertensive intracerebral hemorrhage,and can effectively reduce the serum hs-CRP and Hcy levels.It has a certain clinical efficacy in the treatment of hypertensive intracerebral hemorrhage.
