摘要
目的观察涎腺导管癌(SDC)中人高迁移率族AT Hook蛋白2(HMGA2)和蓬乱蛋白Dsh同源物2(Dvl2)的表达水平,分析其与淋巴结转移、神经侵犯的相关性。方法收集山西医科大学第一医院25例SDC患者的肿瘤组织和20例SDC患者的癌旁涎腺组织,利用免疫组织化学法检测45例标本组织中HMGA2和Dvl2蛋白的表达,采用SPSS21.0统计学软件分析其结果与淋巴结转移和神经侵犯等临床病理学特征之间的关系。结果HMCGA2和Dvl2在SDC中的阳性率分别为56.0%和72.0%,远高于癌旁涎腺组织(阳性率分别为0%和10.0%);在SDC中,HMGA2阳性与淋巴结转移和神经侵犯显著相关(P<0.05),Dvl2阳性表达与淋巴结转移显著相关(P<0.05);HMGA2和Dvl2阳性表达之间存在正相关性(r=0.524,P<0.01);COX单因素回归分析显示,淋巴结转移、神经侵犯和Dvl2阳性影响SDC的预后;但多因素回归分析显示,淋巴结转移和神经侵犯是SDC生存预后的独立预后因素,而Dvl2并不是其独立预后因子。结论HMGA2和Dvl2可能通过Wnt/β-catenin信号通路促进SDC的发生发展,二者的过表达与SDC的神经侵犯和淋巴结转移有关,有望作为SDC未来靶向治疗的潜在靶点。
Objective To study the expression of high-mobility group AT-hook 2(HMGA2)and Dishevelled-2(Dvl2)in salivary duct carcinoma(SDC),and to analyze their correlation with lymph node metastasis and nerve invasion.Methods The tumor tissues of 25 SDC patients and paracancerous salivary gland of 20 SDC patients were collected from the First Hospital of Shanxi Medical University.The expression of HMGA2 and Dvl2 protein in 45 specimens was detected by immunohistochemistry,and its relationships with clinicopathological features such as lymph node metastasis and nerve invasion were analyzed.The results were analyzed by SPSS21.0 software.Results The positive rate of HMGA2 was higher in SDC than in paracancerous salivary glands(56.0%vs 0%).The positive rate of Dvl2 was higher in SDC than in paracancerous salivary glands(72.0%vs 10.0%).In SDC,the positive expression of HMGA2 was significantly correlated with lymph node metastasis and nerve invasion(P<0.05),and the positive expression of Dvl2 was significantly correlated with lymph node metastasis(P<0.05).There was a positive correlation between the positive expression of HMGA2 and Dvl2(r=0.524,P<0.01).COX univariate regression analysis showed that lymph node metastasis,nerve invasion and Dvl2 positive expression influenced the prognosis of SDC.Multivariate regression analysis showed that lymph node metastasis and nerve invasion were independent prognostic factors for the survival and prognosis of SDC,while Dvl2 was not an independent prognostic factor.Conclusion HMGA2 and Dvl2 may promote the occurrence and development of SDC through Wnt/β-catenin signaling pathway.The overexpression of HMGA2 and Dvl2 is related to the nerve invasion and lymph node metastasis of SDC,and they are expected to be potential targets for future targeted therapy of SDC.
