重复经颅磁刺激治疗急性脑梗死后抑郁的临床研究

Clinical study of repetitive transcranial magnetic stimulation in depression after acute cerebral infarction

目的观察重复经颅磁刺激(rTMS)治疗急性脑梗死(ACI)后抑郁的疗效和安全性。方法分析116例ACI后抑郁患者按照随机数字法分为对照组和观察组,每组58例。对照组患者接受常规药物治疗,观察组患者在对照组基础上加用rTMS治疗,连续治疗14d,记录两组患者日常生活活动能力评分(ADL)、抑郁自评量表评分(SDS)、汉密尔顿抑郁量表评分(HAMD)、临床疗效和不良反应等。结果与对照组相比,观察组患者在ADL评分、SDS评分、HAMD评分以及临床疗效上均有所好转,差异有统计学意义(P<0.05)。不良反应中仅观察组患者出现2例口干乏力,休息后好转。结论rTMS治疗ACI后抑郁是有效且相对安全。

Objective To observe the efficacy and safety of repetitive transcranial magnetic stimulation(rTMS)in depression after acute cerebral infarction(ACI).Methods Retrospective analysis of 116 patients with depression after ACI was divided into control group and observation group according to random number method.Patients in the control group received conventional drug therapy,and patients in the observation group received rTMS treatment based on the control group for 14 consecutive days.Scores of daily living activities(ADL),selfrating depression scale(SDS),Hamilton depression scale(HAMD),clinical efficacy and adverse reactions were recorded in the two groups.Results Compared with the control group,the ADL score,SDS score,HAMD score and clinical efficacy were improved in the observation group(P<0.05).Among the adverse reactions,only 2 patients in the observation group showed dry mouth and weakness,improved after rest.Conclusion rTMS is a relatively effective and safe treatment for depression after ACI.
