大承气汤加味干预急性胰腺炎合并腹腔高压患者的临床研究

Clinical Study on Intervention of Modified Dachengqi Decoction on Acute Pancreatitis Complicated with Intra-abdominal Hypertension

目的评价大承气汤加味在治疗急性胰腺炎合并腹腔高压患者中的作用。方法将急性胰腺炎合并腹腔高压患者随机分成对照组和治疗组,对照组参照2014年《急性胰腺炎诊治指南》给予常规治疗,治疗组在对照组基础上加用大承气汤加味。比较两组患者中医症状量化积分,症状、体征、实验室指标好转时间,腹部CT评分,住ICU天数,总住院天数及腹腔内压力变化。结果治疗组在改善患者中西医临床症状及体征,缩短住ICU天数及总住院天数和降低腹内压等方面优于对照组,但在血、尿淀粉酶转阴时间及腹部影像学上未体现明显优势。结论大承气汤加味治疗急性胰腺炎合并腹腔高压患者疗效明显。

Objective:To evaluate the role of Dachengqi Decoction on acute pancreatitis with intra-abdominal hypertension.Methods:Patients were randomly divided into the control group and the treatment group.The control group were given routine treatment according to 2014 Guidelines for the Diagnosis and Treatment of Acute Pancreatitis.The treatment group were added Dachengqi Decoction on that basis.Quantitative scores of TCM symptoms,improvement time of symptoms,signs and laboratory indicators,abdominal CT scores,ICU days,total hospitalization days and intra-abdominal pressure were compared between groups.Results:The treatment group was superior to the control group in improving clinical symptoms and signs,shortening ICU days and total hospitalization days,and reducing intra-abdominal pressure.However,there were no obvious advantages in blood and urine amylase negative conversion time and abdominal imaging.Conclusion:Dachengqi Decoction is effective on acute pancreatitis with intra-abdominal hypertension.
