眩晕2号方治疗后循环缺血性眩晕的临床研究

The Clinical Observation on Ischemic Vertigo after Circulation Treated with Xuanyun 2nd Decoction

目的观察眩晕2号方对后循环缺血性眩晕气血亏虚证患者的临床疗效并探讨其作用机制。方法60例患者随机分为观察组与对照组,均予西医常规治疗,观察组同时给予眩晕2号方治疗,疗程为10 d。观察治疗前后中医证候评分、肿瘤坏死因子-α(TNF-α)、神经元特异性烯醇化酶(NSE)、椎-基底动脉血流TCD参数BA-VS变化。结果两组治疗第10天中医证候分值较治疗第1天均降低(均P<0.01),且观察组治疗第10天均优于对照组治疗第10天(P<0.01)。观察组治疗第10天TNF-α水平、NSE水平均较治疗第1天降低(均P<0.05);而对照组第10天TNF-α水平与治疗前比较,差别不大(P>0.05),NSE水平则升高(P<0.01)。观察组两指标同时段与对照组比较,差异均有统计学意义(均P<0.05)。观察组治疗第10天BA-VS水平较治疗第1天升高(P<0.05),对照组与治疗第1天比较,差别不大(P>0.05)。两组治疗第10天BA-VS水平比较,差别不大(P>0.05)。观察组在改善眩晕等主要证候积分上更有优势,总有效率均高于对照组(均P<0.01)。结论眩晕2号方能够降低患者TNF-α、NSE水平,增加患者的后循环血流,改善患者眩晕症状,提高临床疗效。

Objective:To observe the clinical effect of Xuanyun 2 nd Decoction on patients with posterior circulation ischemic vertigo.Methods:60 patients were randomly divided into the observation group and the control group.Both received conventional Western medicine treatment.The observation group was added Xuanyun 2 nd Decoction.Changes of TCM syndrome scores,TNF-α,NSE,BA-VS changes of vertebrobasilar flow TCD parameters were observed before and after treatment.Results:The scores of TCM syndrome on the 10 th day of treatment in both groups were lower than those on the 1 st day of treatment(P<0.01),and the observation group was better than the control group(P<0.01).The levels of TNF-αand NSE in the observation group were lower than those on the first day of treatment(P<0.05),while the level of TNF-αin the control group showed no significant difference,but NSE level was higher than those before treatment(P<0.01).Compared with the control group,the difference of the two indexes in the observation group was statistically significant(P<0.05).The level of BA-VS in the observation group was higher than that on the first day of treatment(P<0.05),but there was no significant difference in the control group(P>0.05).There was no significant difference in BA-VS between the two groups(P>0.05).Xuanyun 2 nd Decoction could improve the main symptoms of wind phlegm syndrome and dizziness,but it had more advantages in improving the main symptoms of vertigo and the syndrome scores of Qi deficiency,blood deficiency and phlegm blood stasis syndrome.The total effective rate of Xuanyun 2 nd Decoction was higher than that of the control group(all P<0.01).Conclusion:Xuanyun 2 nd Decoction can reduce the levels of TNF-αand NSE,increase the blood flow after circulation and improve the symptoms of vertigo and the clinical efficacy.
