摘要
目的:观察糖肾康颗粒对糖尿病肾病患者PCX、Nephrin、Podocin表达的干预作用。方法:72例糖尿病肾病患者随机分为观察组和对照组各36例,最终完成69例,观察组35例,对照组34例。观察组与对照组均给予基础治疗,观察组加服糖肾康颗粒口服,每日3次,每次10g。两组疗程均为8周。观察两组的临床疗效、中医证候积分变化、治疗前后UACR、24hUPr及尿中PCX、Nephrin、Podocin蛋白的变化情况。结果:观察组临床疗效和中医证候疗效均优于对照组(P<0.05)。两组治疗后中医证候积分、UACR、24hUPr较治疗前均显著下降(P<0.01),但观察组均显著优于对照组(P<0.01)。两组治疗前尿PCX、Nephrin、Podocin水平均高于正常组(P<0.01);治疗后两组尿PCX、Nephrin、Podocin水平均较治疗前降低(P<0.05或P<0.01),但观察组均显著优于对照组(P<0.05)。结论:糖肾康颗粒可有效改善糖尿病肾病患者的临床症状,减少尿蛋白,其作用机制可能与保护足细胞而降低尿中PCX、Nephrin、Podocin蛋白的表达有关。
Objective: To observe the effect of Tangshenkang Granule on the expression of PCX, nephrin and podocin in diabetic nephropathy patients.Methods: 72 cases of DN were randomly divided into 36 cases in the observation group and the control group, and which were completed in 69 cases, 35 in the observation group and 34 in the control group. Both group were given basic treatment, and the observation group took the oral administration of Tangshenkang Granule, three times a day, each time 10 g, and the course of treatment was 8 weeks. The clinical efficacy, the integral of TCM syndrome, UACR, 24 h UPr and the changes of PCX, Nephrin and Podocin in urine were observed. Results: The total effective efficiency of the observation group and the total effective rate of TCM syndrome were better than that of the control group(P<0.05). After treatment, the score of TCM syndrome, UACR and 24 h UPr in the observation group and the control group were decreased(P<0.01), and the decrease in observation group was better than that of the control group(P<0.01). Before treatment, the nephrin urinary, PCX and podocin levels in both of observation group and control group were higher than the normal group(P<0.01)while after treatment, the nephrin urinary, PCX and podocin levels in each group were significantly lower than before treatment(P<0.05 or P<0.01), and the observation group was significantly better than the control group(P<0.05).Conclusion: Tangshenkang Granule can effectively improve the clinical symptoms and reduce the urine protein of DN patients, and the mechanism may be related to inhibiting the expression of PCX, Nephrin and Podocin protein in urine.
