痰热壅肺型急诊社区获得性肺炎患者血清和肽素、肾上腺髓质素原与肺炎评分系统相关性研究

Correlation Between Serum and Copeptin, Adrenomedullin and Pneumonia Scoring System in Patients with Acute Pneumonitis

目的研究急诊社区获得性肺炎患者和肽素(Copeptin)、肾上腺髓质素原(proADM)与肺炎评分系统相关性,探求炎症指标在痰热壅肺证型中的分布规律。方法方便选取该院2018年1—12月60例患者和60名健康对照者,检测Copeptin、proADM,计算PSI、SMART-COP值,进行相关性分析。结果患者炎症指标肺炎评分系统值[Copeptin(58.63±10.27)pmol/L、proADM(539.58±123.45)pmol/L、PSI(134.56±23.55)分、SMART-COP(4.67±1.70)分]均高于健康对照者,差异有统计学意义(t=3.159、3.513、2.997、2.976,P<0.05)。炎症指标与肺炎评分系统值呈正相关,差异有统计学意义(r=0.771、0.821、0.786、0.808,P<0.01)。结论痰热壅肺型急诊CAP炎症指标及肺炎评分高于健康人群。炎症指标值越高,评分越高,病情更重,预后更差。

Objective To study the correlation between the patients with acute community acquired pneumonia and the combination of Copeptin,pro-adrenomedullin(proADM)and pneumonia scoring system,and to explore the distribution of inflammatory markers in the syndrome of phlegm and blood stasis.Methods Convenient election 60 healthy controls and 60 patients in the hospital from January to December in 2018 were enrolled.Copeptin and proADM were detected.PSI and SMART-COP values were calculated and correlation analysis was performed.Results The inflammatory index pneumonia score system values[Copeptin(58.63±10.27)pmol/L,proADM(539.58±123.45)pmol/L,PSI(134.56±23.55)points,SMART-COP(4.67±1.70)points]were higher than those of healthy controls,The difference was statistically significant(t=3.159,3.513,2.997,2.976,P<0.05).The inflammatory index was positively correlated with the pneumonia score system,The difference was statistically significant(r=0.771,0.821,0.786,0.808,P<0.01).Conclusion The index of CAP inflammation and pneumonia in emergency sputum-type pulmonary sputum is higher than that in healthy people.The higher the inflammatory index value,the higher the score,the more severe the condition and the worse the prognosis.
