同型半胱氨酸、超敏C-反应蛋白和D-二聚体在急性脑梗死患者中的应用价值 被引量:3

The Application Value of Homocysteine, High-sensitivity C-reactive Protein and D-dimer in Patients with Acute Cerebral Infarction

摘要 目的探讨血清同型半胱氨酸(HCY)、血浆超敏C-反应蛋白(hs-CRP)和血浆D-二聚体(D-dimer)水平在急性脑梗死(ACI)患者中的应用价值。方法方便选取2017年1月—2018年1月该院收治的已确诊为ACI的90例患者作为试验组,另选择60名健康体检者作为对照组。采用速率法检测二组血清HCY水平,利用干式免疫荧光法对血浆hs-CRP与D-dimer水平进行测定,并比较这些数据在不同组别中的差异。结果试验组血清HCY(20.15±6.05)μmol/L、血浆hs-CRP(8.98±2.95)mg/L和血浆D-dimer(495.19±110.16)μg/L均显著高于对照组[血清HCY(6.68±2.85)μmol/L、血浆hs-CRP(2.81±1.08)mg/L和血浆D-dimer(238.75±49.93)μg/L],差异有统计学意义(t=24.124、22.520、19.512,P<0.05);非小面积梗死组血清HCY(27.12±3.93)μmol/L、血浆hs-CRP(10.38±3.09)mg/L和血浆D-dimer(555.20±120.31)μg/L显著高于小面积梗死组[血清HCY(14.56±2.35)μmol/L、血浆hs-CRP(7.96±2.15)mg/L和血浆D-dimer(440.19±99.52)μg/L],差异有统计学意义(t=14.357、9.387、12.566,P<0.05),表明脑梗死的面积与血清HCY、血浆hs-CRP和血浆D-dimer呈正相关。结论血清HCY、血浆hs-CRP和血浆D-dimer与ACI的病情严重程度密切相关,对病情的评估及预后判断具有一定的应用价值。 Objective To investigate the application of serum homocysteine(HCY),plasma high-sensitivity C-reactive protein(hs-CRP)and plasma D-dimer(D-dimer)levels in patients with acute cerebral infarction(ACI)value.Methods 90 patients with ACI diagnosed in the hospital from January 2017 to January 2018 were Conveniently selected as the experimental group,and 60 healthy subjects were selected as the control group.The rate of HCY in the two groups was measured by the rate method.The levels of plasma hs-CRP and D-dimer were measured by dry immunofluorescence,and the differences of these data in different groups were compared.Results Serum HCY(20.15±6.05)μmol/L,plasma hs-CRP(8.98±2.95)mg/L and plasma D-dimer(495.19±110.16)μg/L were significantly higher in the experimental group than in the control group[serum HCY(6.68±2.85)μmol/L,plasma hs-CRP(2.81±1.08)mg/L and plasma D-dimer(238.75±49.93)μg/L],the difference was statistically significant(t=24.124,22.520,19.512,P<0.05);Serum HCY(27.12±3.93)μmol/L,plasma hs-CRP(10.38±3.09)mg/L and plasma D-dimer(555.20±120.31)μg/L were significantly higher in the non-small infarction group than in the small infarct group[Serum HCY(14.56±2.35)μmol/L,plasma hs-CRP(7.96±2.15)mg/L and plasma D-dimer(440.19±99.52)μg/L],the difference was statistically significant(t=14.357,9.387,12.566,P<0.05),indicating that the area of cerebral infarction was positively correlated with serum HCY,plasma hs-CRP and plasma D-dimer.Conclusion Serum HCY,plasma hs-CRP and plasma D-dimer are closely related to the severity of ACI,and have certain application value for disease assessment and prognosis.
