腹腔镜卵巢囊肿剥除术中缝合止血与电凝止血对卵巢功能的影响

Effect of Suture Hemostasis and Electrocoagulation on Ovarian Function during Laparoscopic Ovarian Cystectomy

目的探讨腹腔镜卵巢囊肿剥除术中缝合止血与电凝止血对卵巢功能的影响。方法回顾性分析2016年3月-2018年3月该院收治的卵巢囊肿患者64例的临床资料,按照术中止血方式不同分成缝合组(缝合止血,32例)和电凝组(电凝止血,32例),比较两组卵巢功能及手术指标。结果电凝组血清雌二醇(E2)(125.37±11.41)pg/mL及手术时间(42.35±9.61)min、术中出血量(58.74±11.54)mL较缝合组低[(156.24±10.32)pg/mL、(54.12±8.79)min、(81.63±10.92)mL],术后引流量(68.34±10.83)mL、黄体生成素(LH)(9.42±1.69)mIU/mL、卵泡刺激素(FSH)(4.53±1.12)IU/L高于缝合组[(57.32±11.45)mL、(5.34±1.13)m IU/mL、(2.85±1.06)IU/L],差异有统计学意义(t=11.351、5.112、8.150、3.955、11.353、6.163,P<0.05)。结论缝合止血较电凝止血在行腹腔镜卵巢囊肿剥除术中止血效果较为理想,虽其手术时间较长、术中出血量较多,但其对卵巢功能损伤较少。

Objective To investigate the effect of suture hemostasis and electrocoagulation on ovarian function during laparoscopic ovarian cystectomy. Methods The clinical data of 64 patients with ovarian cysts admitted to the hospital from March 2016 to March 2018 were retrospectively analyzed. According to the different methods of intraoperative hemostasis, they were divided into suture group(suture hemostasis, 32 cases) and electrocoagulation group(electrocoagulation hemostasis, 32 cases), comparing the ovarian function and surgical indicators of the two groups. Results Serum estradiol(E2)(125.37 ±11.41) pg/mL and operation time(42.35 ±9.61) min,intraoperative blood loss(58.74±11.54)mL were lower in the electrocoagulation group than in the suture group[(156.24±10.32) pg/m L,(54.12±8.79) min,(81.63±10.92)mL], postoperative drainage(68.34±10.83) mL, luteinizing hormone(LH)(9.42±1.69) mIU/mL, follicle stimulating hormone(FSH)(4.53 ±1.12) IU/L was higher than suture group [(57.32±11.45) mL,(5.34±1.13) m IU/mL,(2.85±1.06)IU/L],the difference was statistically significant(t =11.351, 5.112, 8.150, 3.955, 11.353, 6.163, P <0.05). Conclusion Suture hemostasis is more effective than electrocoagulation for hemostasis in laparoscopic ovarian cystectomy. Although it has longer operation time and more intraoperative blood loss, it has less damage to ovarian function.
