摘要
目的研究在硬核白内障超声乳化手术中挤压核、压核块技术的应用效果。方法以2018年1月-2019年7月该院收治的硬核白内障168例患者为对象,按照随机数字表法分为两组,84例患者接受水平劈核技术行白内障超声乳化手术,84例患者应用挤压核、压核块技术实施硬核白内障超声乳化手术,相应视作对照组、观察组,比较两组手术情况。结果观察组超声乳化用时为(59.34±12.16)s,超声能量使用率(8.14±3.37)%,均低于对照组(145.72±24.19)s、(20.72±7.49)%,差异有统计学意义(t=29.241、14.038,P<0.05);观察组术后1个月角膜内皮细胞计数为(2 897.46±293.45)个/mm^2,多于对照组(2 643.31±215.16)个/mm^2,角膜内皮细胞丢失率为(4.51±2.34)%,低于对照组(9.58±3.37)%,差异有统计学意义(t=6.401、11.326,P<0.05);观察组术后并发症发生率为4.76%,低于对照组发生率14.29%,差异有统计学意义(χ^2=4.421,P<0.05)。结论在硬核白内障超声乳化手术中应用挤压核、压核块技术可缩短手术用时,提升手术安全性,有良好应用价值。
Objective To study the application effect of extrusion nucleus and nucleus block technique in phacoemulsification of hard cataract. Methods A total of 168 patients with hard cataract from January 2018 to July 2019 in the hospital were enrolled in the study.They were randomly divided into two groups. 84 patients underwent phacoemulsification with horizontal nucleus nucleus and 84 patients underwent phacoemulsification. The nucleus cataract technique was performed by pressure nucleus and pressure nucleus technique, which was regarded as the control group and the observation group. The operation of the two groups was compared. Results The phacoemulsification time of the observation group was(59.34±12.16) s, and the ultrasonic energy utilization rate was(8.14±3.37)%,which was lower than that of the control group(145.72 ±24.19) s and(20.72±7.49)%,the difference was statistically significant(t=29.241, 14.038, P<0.05);the corneal endothelial cell count in the observation group was(2 897.46±293.45)cases/mm^2 at 1 month after operation, more than the control group(2643.31±215.16)cases/mm^2, and the corneal endothelial cell loss rate was(4.51±2.34)%, lower than the control group(9.58±3.37)%,the difference was statistically significant(t=6.401, 11.326, P<0.05);the postoperative complication rate was 4.76% in the observation group, 14.29% lower than the control group,the difference was statistically significant(χ^2=4.421, P<0.05). Conclusion The application of squeezed nucleus and nucleus block technique in hard phacoemulsification can shorten the time of surgery and improve the safety of surgery. It has good application value.
