NRS 2002筛查高龄COPD住院患者营养风险及与体格检查指标的相关性

Correlation between application of NRS 2002 and indicators of physical examination in elderly patients with inpatients with COPD

目的探讨高龄住院慢性阻塞性肺疾病(COPD)患者营养风险筛查应用营养风险筛查2002(NRS 2002)与体格检查指标的相关性。方法选取我院2018年1月至2019年4月收治的高龄COPD住院患者120例作为研究对象,给予NRS 2002营养风险筛查与体格检查,根据对应指标分析NRS 2002筛查结果与体格检查指标的相关性。结果经NSR 2002筛查,营养不良患者50例(41.67%),不同性别患者营养风险评分比较差异无统计学意义(P>0.05),不同肺功能分级与不同年龄患者营养风险评分差异具有统计学意义(P<0.05);体格指标检查不同性别患者肱二头肌皮褶厚度(TSF)、上臂肌围(MAMC)、去脂体质量指数(FFMI)等指标具有统计学意义,不同年龄患者BMI存在统计学意义(P<0.05),≥80岁高龄患者BMI明显低于70~79岁者;BMI,FFMI与NRS 2002评分存在关联性(r=0.510,0.370,P<0.05)。结论高龄COPD患者发生营养风险概率极高,临床可应用NRS 2002与体格检查指标结合的方式对其进行综合全面的营养评估。

Objective To discuss the correlation between application of nutrition risk screening 2002(NRS 2002)and indicators of physical examination in elderly patients with inpatients with chronic obstructive pulmonary diseases(COPD).Methods To select 120 cases of elderly patients with COPD admitted to our hospital from January 2018 to April 2019 as the study subjects.and NRS 2002 and physical examination were given to them.The correlation between results of NRS 2002 and indicators of physical examination was analyzed according to corresponding indicators.Results The NSR 2002 showed that there were 50 cases(41.67%)of malnourished patients,and there was no significant difference in nutritional risk scores between patients of different genders(P>0.05).The difference in nutritional risk scores between different lung function grades and patients of different ages was statistically significant(P<0.05).The indexes of physical examination such as skinfold thickness of biceps brachii(TSF),midarm muscle circumference(MAMC),and fat-free mass index(FFMI),etc.were statistically significant between patients of different genders,and the BMI of patients of different ages was statistically significant(P<0.05).The BMI of patients aged≥80 years was significantly lower than those of 70 to 79 years;There was a correlation between MBI,FFMI and NRS 2002 scores(r=0.510,0.370,P<0.05).Conclusion The elderly patients with COPD had a very high probability of nutritional risk.The combination of NRS 2002 and physical examination indicators could be used to comprehensively assess their nutrition.
