窝沟封闭术用于儿童第一恒磨牙龋病的临床效果分析

Analysis of the clinical effect of pit and fissure closure in children’s first permanent molar caries

目的总结窝沟封闭术在儿童第一恒磨牙龋病(dental caries)中的应用效果。方法选择2015-3—2016-12期间在邵阳学院附属第二医院口腔科进行第一恒磨牙窝沟封闭的98名(242颗)儿童临床资料,按照随机原则分为两组各49名,对照组儿童未接受医疗处理,仅予以口腔卫生宣教等干预,观察组儿童则接受窝沟封闭术医疗干预。比较两组对象干预后6月、12月、18月第一恒磨牙龋病发生率,同时统计观察组儿童第一恒磨牙窝沟封闭剂脱落情况。结果(1)观察组第一恒磨牙龋病干预后6月发生率(0)、12月发生率(0.83%)、18月发生率(2.50%)明显低于对照组分别为4.91%、6.55%和10.65%,差异有统计学意义(P<0.05)。(2)进一步对观察组检查显示,本组儿童第一恒磨牙窝沟封闭剂6月脱落率(1.67%),12月脱落率(2.56%),18月脱落率(3.48%),由此可见窝沟封闭剂可随着时间延长逐渐递减。结论窝沟封闭术应用于儿童第一恒磨牙中确有较好效果,有助于减少儿童第一恒磨牙龋病的发生,随着窝沟封闭剂的保留时间延长,可进一步增加远期疗效,值得临床研究。

Objective To summarize the application effect of pit and fissure closure in children s first permanent dental caries.Methods The clinical data of 98 children(242)with first permanent detal caried by pit and fissure closure were selected in the Department of Stomatology of our hospital during the period of 2015-03—2016-12,and randomly divided into two groups(49 casesin each group).The control group did not receive medical treatment,but only received oral hygiene education,and the observation group received medical intervention for pit and fissure closure.The incidence of first permanent dental rickets at 6 months,12 months,and 18 months after intervention in the two groups was compared,and the loss of the first permanent molars in children in the observation group was also counted.Results(1)In the observation group,the incidence of first permanent at 6 months(0),12 months(0.83%),and 18 months(2.50%)after intervention was significantly lower than that in the control group 4.91%,6.55%,and 10.65%,with statistically significant differences(P<0.05).(2)further examination of the observation group shows that the abscission rate of the first permanent molars at 6 months(1.67%),12 months(2.56%)and 18 months(3.48%),indicating that the pit and fissure sealant can gradually decrease with the extension of time.Conclusion Pit and fissure closure has a good effect in children s first permanent molars,which can help to reduce the incidence of the first permanent molars in children,with the prolongation of the retention time of pit and fissure sealants,the long-term efficacy can be further increased,which is worthy of clinical application.
