临床综合护理对预防双侧腹股沟疝患者术后出血及护理满意度的影响

Effect of clinical comprehensive nursing on postoperative bleeding prognosis and nursing satisfaction in patients with bilateral inguinal hernia

目的探讨临床综合护理对预防双侧腹股沟疝患者术后出血及护理满意度的影响。方法选取2015年1月至2019年6月,江苏省人民医院溧阳分院收治的68例双侧腹股沟疝患者为研究对象。采用随机数字表法将其分为对照组和研究组,每组患者34例。其中对照组采用常规护理,研究组采用综合护理。2组均连续护理2周,随访1个月。比较2组患者术后疼痛视觉模拟评分(VAS)、胃肠功能及日常生活恢复时间、预后情况及护理满意度。结果术后1、3 d,研究组VAS评分分别为(4.82±1.08)分、(2.76±0.94)分,对照组分别为(4.73±0.92)分、(3.19±1.06)分,差异无统计学意义(t=0.610、3.437,P=0.438、0.068);术后5~7 d,2组VAS评分均呈逐渐降低趋势,且术后5、7 d研究组的VAS评分分别为(2.18±0.79)分、(1.54±0.46)分,均低于对照组(2.73±0.85)分、(2.26±0.61)分,差异有统计学意义(P<0.05)。研究组术后胃肠功能恢复、术后住院及日常生活恢复时间分别为(1.04±0.29)分、(7.85±2.42)分、(16.59±3.74)分,对照组术后胃肠功能恢复、术后住院及日常生活恢复时间分别为(1.25±0.37)分、(9.36±3.15)分、(20.82±4.13)分,差异均有统计学意义(P<0.05)。研究组并发症发生率为2.94%,对照组并发症发生率为9.09%,研究组显著低于对照组,差异有统计学意义(χ^2=2.356,P<0.05)。研究组护理满意度100.00%,对照组护理满意度79.41%,2组比较差异有统计学意义(χ^2=5.733,P=0.017)。结论临床综合护理,能够明显减轻双侧腹股沟疝术后出血及患者疼痛症状,缩短胃肠功能恢复时间、术后住院时间及日常生活恢复时间,并能显著提高患者对护理的满意度。

Objective To investigate the effect of clinical comprehensive nursing on postoperative bleeding and nursing satisfaction of patients with bilateral inguinal hernia.Methods A total of 68 patients with bilateral inguinal hernia admitted to Jiangsu Provincial People's hospital between January 2015 and June2019 were enrolled.They were divided into the conventional group and the study group by random number table method,with 34 cases in each group.All were followed up for 1 month.The conventional group used routine nursing,the study group used comprehensive nursing,and the two groups of patients were continuously treated for 2 weeks.The scores of visual analogue scale(VAS),gastrointestinal function and daily activity recovery time,prognosis and nursing satisfaction were observed and compared between the two groups.Results At 1 day and 3 day after surgery,the VAS scores of the study group were(4.82±1.08)and(2.76±0.94),respectively,and the control group was(4.73±0.92)and(3.19±1.06),with no significant difference(P>0.05);the scores of VAS in both groups showed a gradual decrease trend at 5 to 7 days after surgery;and the scores in study group at 5 and 7 days after surgery were(2.18±0.79)and(1.54±0.46),respectively,significantly lower than the control group,(2.73±0.85)and(2.26±0.61)points,and the difference was statistically significant(P<0.05).Gastrointestinal function recovery time,postoperative hospitalization and daily activity recovery time in the study group were(1.04±0.29)points,(7.85±2.42)points,and(16.59±3.74)points,all significantly shorter than those in the control group(1.25±0.37)points,(9.36±3.15)points,and(20.82±4.13)points,the differences were statistically significant(P<0.05).The incidence of complications in the study group was 2.94%,which was lower than that in the conventional group(9.09%,P<0.05).The nursing satisfaction in the study group was 100.00%,which was obviously higher than that in the conventional group(79.41%,P<0.05).Conclusion Clinical comprehensive nursing can significantly alleviate the pain symptoms of patients and bleeding after bilateral inguinal hernia,shorten the recovery time of gastrointestinal function,postoperative hospital stay and daily activity recovery time,and can significantly improve patient nursing satisfaction.
