分娩镇痛对瘢痕子宫再次妊娠经阴道顺产产程的影响

Effect of labor analgesia on labor progress of vaginal birth after previous C-section

目的探讨分娩镇痛对瘢痕子宫再次妊娠产妇经阴道顺产产程的影响。方法以瘢痕子宫再次妊娠经阴道试产的产妇为研究对象,选取2016-01/2018-12期间我院产房具备阴道试产条件并经阴道顺产产妇共738例,按是否行分娩镇痛分为镇痛组(n=334)和非镇痛组(n=404),比较2组产妇产程的差异性。结果镇痛组总产程与非镇痛组比较,差异有统计学意义(P<0.05),其中第一产程、第二产程,2组比较差异有统计学意义(P<0.05),而第三产程比较,差异无统计学意义(P>0.05);2组比较,新生儿Apgar 1 min、5 min、10 min评分,差异无统计学意义(P>0.05)。结论分娩镇痛会明显延长瘢痕子宫再次妊娠产妇的第一产程及第二产程,但是产程的延长并不影响新生儿的妊娠结局。

Objective To investigate the effect of labor analgesia on labor progress of vaginal birth after previous C-section.Methods A total of 738 puerperas undergoing vaginal birth after previous C-section in Maternal and Child Health Center of Guangxi Zhuang Autonomous Region from January 2016 to December 2018 were included and divided into analgesia group(n=334)and non-analgesia group(n=404),according to whether labor analgesia was performed.The relevant indicators of puerperas in both groups were compared.Results The total labor stage of the analgesia group was significantly different from that of the non-analgesia group(P<0.05).And there were significant differences in the first and second labor stages between the two groups(P<0.05),while the difference of the third labor stage was not statistically significant(P>0.05).There was no significant difference in Apgar scores between the two groups at 1 min,5 min and 10 min after birth(P>0.05).Conclusion Labor analgesia can significantly prolong the first and second stages of labor in puerperas undergoing vaginal birth after previous C-section,but the prolonged stage of labor does not affect the outcome of pregnancy.
