家庭医生签约服务试点困境及对策——以咸宁市为例

Difficulties and Countermeasures of Family Doctor Contract Service——Based on the Research of Xianning

目的了解家庭医生签约服务落实状况、家庭医生团队建设、行政主管部门制度保障等方面,以期针对家庭医生签约服务遇到的困难,寻找相应的原因并提供解决对策。方法以咸宁市3个区6个社区的卫生服务中心和一家市中医医院及2家养老院的家庭医生签约服务人群和相关负责人为调查对象,开展问卷调查。结果家庭医生签约服务签约率高,总体实施情况较好,但也存在一些困境:一是居民对家庭医生认知度不高,二是家庭医生数量不足,三是家庭医生工作量大,薪酬少,四是相应的法律保障不完善。结论家庭医生签约服务有待进一步完善,可从以下几方面着手:一是加大宣传力度,二是加强人才培养,三是改革收入分配机制,四是完善法律保障。

Objective To understand the implementation of family doctor contract service,family doctor team building,administrative department system guarantee and other aspects,with a view to the family doctor contract service encountered difficulties,find the corresponding reasons and provide some solutions.Methods Field visits were conducted to the health service centers in 6 communities in 3 districts of Xianning city,the contracted service groups of family doctors in a municipal hospital of traditional Chinese medicine and 2 nursing homes and the relevant persons in charge.Results The signing rate of family doctors was high,and the overall implementation was good,but there were also some difficulties:first,residents did not have a high awareness of family doctors;second,the number of family doctors was insufficient;third,the workload of family doctors was large,the salary was small;fourth,the corresponding legal protection was not perfect.Conclusion The contracted services of family doctors need to be further improved,which can be started from the following aspects:first,to strengthen publicity,second,to strengthen personnel training,third,to reform the income distribution mechanism,fourth,to improve the legal guarantee.
