摘要
准确的淋巴结分期是评估非小细胞肺癌的重要组成部分,其与临床决策及疾病的预后密切相关。而正电子发射计算机断层显像(PET)/CT因兼具解剖与功能成像以及无创两大特点被应用于评估非小细胞肺癌淋巴结转移。其中,最大标准摄取值是诊断淋巴结状态的常用参数。肺癌原发灶的生理特性因与淋巴结转移密切相关,故其代谢参数可较好地预测淋巴结转移。与氟代脱氧葡萄糖显像剂不同,代表细胞增殖与血管生成的显像剂更有助于体现肿瘤的生物特征,进一步判断肺癌淋巴结转移。另外,影像组学可以提取更多可靠的淋巴结图像特征,且应用机器学习模型能更准确地判断淋巴结转移。未来,PET/CT影像组学的发展将推动人们更精准地诊断淋巴结转移。
Accurate lymph node staging is an important part of assessing non-small cell lung cancer,which is closely related to the clinical decision making and disease prognosis.Positron emission tomography(PET)/CT has been used to evaluate lymph node metastasis in non-small cell lung cancer due to its anatomical and functional imaging characteristics and non-invasive characteristics.The maximum standard uptake value is a common parameter for the diagnosis of lymph node status.The physiological characteristics of primary lung cancer are closely related to lymph node metastasis,so the metabolic parameters can better predict the lymph node metastasis.Different from fluorodeoxyglucose imaging agents, imaging agents representing cell proliferation and angiogenesis are more helpful to reflect the biological characteristics of tumors and further identify lymph node metastasis of lung cancer.In addition,imagingomics can extract more reliable lymph node image features,and the application of machine learning model can more accurately judge lymph node metastasis.In the future,the development of PET/CT radiomics will enable more accurate diagnosis of lymph node metastasis.
