高血压合并2型糖尿病的治疗进展

Therapy in Hypertensive Patients with Type 2 Diabetes Mellitus

高血压是心血管疾病的主要危险因素,尤其是对于高血压病合并2型糖尿病的患者,两种疾病并发可显著增加患者心、脑、肾等靶器官损害风险,有效降低高血压和控制高血糖是降低心血管远期严重并发症的发生和降低该类患者病死率的关键。血管紧张素转换酶抑制剂和血管紧张素Ⅱ受体阻滞剂是大多数指南推荐的首选降压药物,二甲双胍、胰高血糖素样肽-1受体激动剂、钠-葡萄糖同向转运蛋白2抑制剂等降糖药物有一定协同降压作用。现简要阐述目前针对高血压合并2型糖尿病的患者在治疗上的进展。

Hypertension is a major risk factor for cardiovascular disease,especially in patients with hypertension and diabetes mellitus.Risk in cardio-cerebral vascular complication significantly increased in patients with both this combination.Therefore,the management of elevated blood pressure is a critical component of the comprehensive clinical management of diabetics.Guidelines recommended ACE inhibitors or ARBs as initial therapy for hypertension patients with diabetes mellitus.New antidiabetic drugs,such as GLP-1 receptor agonist,DPP-4 inhibitors and SLGT-2 inhibitors,may have positive effects on blood pressure.The article reviews the progress of therapy in hypertensive patients with diabetes mellitus.
