人工全膝关节表面置换手术并发症的回顾性分析与探讨

Retrospective analysis and discussion on the complications of total knee arthroplasty

目的:探讨人工膝关节表面置换术中、术后并发症的预防。方法:2010年1月-2017年12月收治行人工膝关节置换患者380例,分析并发症及采取的相应处理措施,评价患者手术前后KSS评分。结果:出现并发症12例,其中术中股骨髁骨折4例,术后康复髌骨骨折1例,摔伤致髌骨骨折1例;术中侧副韧带损伤2例,腘肌腱损伤2例;术后假体周围感染2例;经术中采取正确处理,恢复满意。假体周围感染2例院外就医诊治。术后随访患者KSS评分较术前明显好转,效果满意。结论:人工膝关节置换患者,经术前详尽准备,术中、术后并发症给予积极、正确的处理及合理的康复锻炼,术后严格管理,可使患者膝关节功能恢复至满意的效果。

Objective:To explore the prevention of complications during and after the total knee arthroplasty.Methods:380 patients with knee arthroplasty were selected from January 2010 to December 2017.We analyzed the complications and the corresponding treatment measures,and evaluated the KSS score of patients before and after operation.Results:There were 12 cases of complications,including 4 cases of femoral condyle fracture,1 case of postoperative tibiofibular fracture,1 case of tibiofibular fracture caused by fall,2 cases of collateral ligament injury and 2 cases of diaphragmatic tendon injury,2 cases of infection around the prosthesis.After the correct treatment during the operation,the recovery was satisfactory.Two cases of periprosthetic infection were treated outside hospital.The KSS score of the follow-up patients after operation was significantly better than that before operation,and the effect was satisfactory.Conclusion:Through detailed preparation before operation,positive and correct treatment and reasonable rehabilitation exercise for intraoperative and postoperative complications,and strict management after operation,the knee function of patients with total knee arthroplasty can be restored to a satisfactory effect.
