三维斑点追踪技术评价左室射血分数正常的肥厚型心肌病患者左心室心肌收缩特性研究 被引量:4

摘要 目的:采用三维斑点追踪技术(3D-STI)评价左室射血分数(LVEF)正常的肥厚型心肌病(HCM)患者左室心肌收缩功能变化。方法:2018年1月-2019年3月收治LVEF≥50%的HCM患者33例作为HCM组,选取正常者33例作为对照组。两组均行常规超声心动图和3D-STI测量,获得室间隔厚度(IVSD)、LVEF、左室整体长轴应变(GLS)、左室整体环向应变(GCS)、左室整体径向应变(GRS)、左室整体面积应变(GAS)及三维应变(3D-strain)。结果:与对照组相比,HCM组IVSD、GCS显著增加,GLS、GRS及3D-strain显著降低,差异有统计学意义(P<0.01)。GLS与IVSD呈正相关性,GCS、GRS及3D-strain与IVSD呈负相关性。结论:3D-STI能够准确评价LVEF正常的HCM患者左心室心肌收缩特性,为临床及早进行治疗干预提供依据。 Objective:To evaluate left ventricular systolic function in patients with hypertrophic cardiomyopathy with normal left ventricular ejection fraction by three-dimensional speckle tracking technique.Methods:33 patients with LVEF≥50%hypertrophic cardiomyopathy were selected as the HCM group from January 2018 March 2019,33 normal subjects were selected as the control group,routine echocardiography and 3D-STI measurements were performed in both groups to obtain ventricular septal thickness(IVSD),left ventricular ejection fraction(LVEF),left ventricular global long axis strain(GLS),left ventricular global circumferential strain(GCS),left ventricular global radial strain(GRS),left ventricular global area strain(GAS),and three-dimensional strain(3D-strain).Results:Compared with the control group,the IVSd and GCS of the HCM group increased significantly,and the GLS,GRS and 3D-strain decreased significantly,the difference was statistically significant(P<0.01).GLS was positively correlated with IVSD,and GCS,GRS and 3D-strain were negatively correlated with IVSD.Conclusion:3D-STI can be accurately evaluated and provide a basis for clinical early intervention.
