生长抑素治疗急性淋巴细胞白血病患儿培门冬酶相关性胰腺炎疗效观察 被引量:5

摘要 目的:探讨生长抑素治疗培门冬酶相关性胰腺炎的临床效果。方法:选取2015年1月至2017年1月我院收治的13例应用CCCG-2015方案化疗后发生培门冬酶相关性胰腺炎的急性淋巴细胞白血病患儿,患儿确诊培门冬酶相关性胰腺炎后使用生长抑素治疗,观察患儿腹部压痛消失时间及血液淀粉酶、血液脂肪酶、影像学恢复正常时间。结果:13例患儿均出现腹痛症状,在使用生长抑素后腹痛症状均缓解,其中1例患儿为坏死性胰腺炎,出现严重休克伴发腹腔继发感染,予以抗感染及补液,转至外科行手术引流后痊愈。治疗后,患儿的生化指标如血液淀粉酶及脂肪酶水平显著降低。结论:生长抑素在培门冬酶相关性胰腺炎中疗效显著,能够显著改善腹痛以及生化指标水平,值得临床推广应用。 Objective:To investigate the clinical efficacy of somatostatin in the treatment of acute lymphoblastic leukemia children with pegaspargase associated pancreatitis.Methods:From Jan.2015 to Jan.2017,a total of 13 children with acute lymphoblastic leukemia who developed pegaspargase associated pancreatitis after chemotherapy of CCCG-2015 regimen in our hospital were selected.Children were treated with somatostatin after diagnosis of pegaspargase associated pancreatitis.Disappearance time of abdominal tenderness,blood amylase,blood lipase and recovery time of imaging were observed.Results:The symptoms of abdominal pain in 13 children were relieved after treatment of somatostatin.One child had necrotizing pancreatitis with severe shock accompanied by abdominal secondary infection,after treatment of anti-infection and fluid infusion,the child was transferred to surgery for surgical drainage and recovered.After treatment,children’s biochemical indicators such as blood amylase and lipase were significantly reduced.Conclusion:The efficacy of somatostatin in the treatment of acute lymphoblastic leukemia children with pegaspargase associated pancreatitis is significant,which can effectively relieve patient’s abdominal pain and biochemical indicators.It is worthy of clinical application.
