前列腺2μm激光切除术后尿道创伤修复的实验研究

Experimental study of prostatic urethral wound repair after 2μm laser prostatectomy

目的探究前列腺2μm激光切除术后前列腺部尿道创伤修复的病理组织学改变及其意义。方法选择成年雄性中华田园犬15只,随机分为5组,所有实验动物均进行2μm激光前列腺切除术,术后按照时间不同进行取材,样本进行HE染色、免疫组织化学染色。结果术后3天,前列腺部尿道创面未见上皮被覆,术后1周可见增生的前列腺基底细胞出现迁移,逐渐开始覆盖创面,出现再上皮化,术后2周在上皮化过程逐渐完成,术后3~4周尿路上皮分化基本完成,肉芽组织成熟,纤维组织增生。免疫组织化学结果均有示:Ki-67(+)、PCNA(+)、p63(+)、PSA(-)、尿斑蛋白(-)。结论2μm激光前列腺切除术后前列腺部尿路创面修复的细胞主要来源于前列腺基底细胞,残余的前列腺组织增殖,并向创面迁移。

Objective To investigate the histopathological changes and clinical significance of prostatic urethral wound repair after 2μm laser prostatectomy.Methods Fifteen adult male Chinese idyllic dogs were randomly divided into five groups according to the time of sampling.They were divided into three groups.All experimental animals underwent 2μm laser prostatectomy.Samples were taken according to the time after operation.HE staining and immunohistochemical staining were performed.Results Three days after operation,no epithelial covering was found in the urethral wound of the prostate.One week after operation,the proliferated prostatic basal cells migrated,gradually began to cover the wound and appeared re-epithelialization.The process of epithelialization was gradually completed 2 weeks after operation.After 3~4 weeks,the differentiation of urinary epithelium was basically completed,granulation tissue matured and fibrous tissue proliferated.Immunohistochemical results showed Ki-67(+),PCNA(+),p63(+),PSA(-),urinary plaque protein(-).Conclusion After 2μm laser prostatectomy,the prostatic urinary tract wound repair cells mainly come from the prostatic basal cells,and the residual prostatic tissue proliferates and migrates to the wound.
