某医院药占比管控下的合理用药情况分析

Analysis of rational drug use under the control of drug proportion in a hospital

目的:分析某医院在控制药占比前后,医院药占比及合理用药的情况变化,为促进合理用药提供依据。方法:利用PDCA循环管理法,通过计划(P)、执行(D)、检查(C)、处理(A)4个环节对该院不合理用药进行控制,并整理前后药占比及辅助用药成效分析、评价。结果:医院于2018年利用PDCA模式对药占比进行管控,2017年和2018年全年药占比分别为31.81%、27.04%,同比下降4.77%,差异显著(P<0.05),较2017年各季度药占比均呈下降趋势。全年辅助药占比分别为8.56%、3.14%,同比下降5.42%,差异显著(P<0.05),各季度辅助药占比均亦呈下降趋势。对比2017年与2018年使用排名前10位的辅助药品的使用金额、使用频次及平均日费用,2018年总体情况均呈下降趋势。结论:通过PDCA循环对医院药占比进行管控成效显著,对医院的辅助用药管理及合理化用药起到明显的促进作用。

Objective:To provide a basis for promoting rational drug use,the changes of drug proportion and rational drug use in hospital were analysed before and after the control of drug proportion. Methods:PDCA cycle management method was used to control unreasonable drug use in the hospital through four links,plan(P),execution(D),inspection(C)and treatment(A),to analyse and evaluate the ratio of drugs before and after treatment and the effect of auxiliary drugs. Results:The hospital adopted PDCA mode to control the drug proportion in 2018. The proportion of drugs in 2017 and 2018 was 31.81% and 27.04% respectively,with a year-onyear decrease of 4.77%,showing a significant difference(P<0.05). The proportion of drugs in each quarter showed a decreasing trend compared with that in 2017. The proportion of auxiliary drugs in 2017 and 2018 was 8.56% and 3.14% respectively,with a year-onyear decrease of 5.42%,showing a significant difference(P <0.05). The proportion of auxiliary drugs in each quarter also showed a decreasing trend compared with that in 2017. Comparing the amount,frequency and average daily cost of the top 10 auxiliary drugs used in 2017 and 2018,the overall situation in 2018 showed a downward trend. Conclusion:PDCA cycle has achieved remarkable results in the control of the proportion of drugs in hospitals,which plays a significant role in promoting the management of auxiliary drugs and rational drug use.
